Job Description:
The Nurse Practitioner- Nursing Home Service functions as a member of our primary care provider and nursing home in the delivery of comprehensive health care services to our patients in the nursing home setting, under the collaboration of a licensed physician. This Nurse Practitioner is a skilled member of the allied health care team, who is expected to practice at the highest level of their licensure. This requires providers to stay up to date with the latest evidence based medicine. This individual works collaboratively with physicians, nurses, case managers and other health care disciplines utilizing a team approach.
In this role, you will spend 80% of your time serving patients in three area nursing homes as well as the hospice program. Additionally, you will fill-in at primary care clinics to cross cover for provider absences.
JOB QUALIFICATIONS EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational
qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: A graduate of an approved Nurse Practitioner educational program. Master’s degree in nursing.
Preferred/Optional: none
EXPERIENCE
Minimum Required: Some previous nursing experience in a hospital setting.
Preferred/Optional: Nurse practitioner experience in a hospital or nursing home setting is preferred. Or two to three years’ experience in a Nursing Home or hospital setting, as a staff nurse is preferred.
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: Current Wisconsin RN Licensure, Nurse Practitioner Certification by a national certifying body. Advanced Practice Nurse Prescriber Certification. Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire. State of Wisconsin valid driver’s license with acceptable driving record.
Preferred/Optional: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
