Homelake Veterans Center
The Colorado State Veterans Center at Homelake is a State-owned and operated skilled nursing facility and domiciliary dedicated to serving those who have served us. We strive to provide excellent customer service and to respect and honor resident choices regarding bathing, rising, bedtimes, and other personal decisions. Our philosophy is to provide residents a home where life and love abound. As a Nurse II. this position exists to provide professional nursing care through cooperation/collaboration with other disciplines, (i.e. M.D., Social Work, Nursing Staff, Rehab. Staff, etc. ) This position provides direct and indirect care to residents. This position administers medications, performs treatments and technical nursing skills and completes documentation meeting V.A. and State regulations.
This position is responsible for the following job duties:
Direct Care and Safety
- Assess, plan implement and evaluate direct care to residents on a skilled nursing care unit or also to include resident care on a Domiciliary unit.
- Coordinate duties as appropriate for specific disciplines and monitors work progress and completion i.e.: RN, LPN, C.N.A.
- Monitor resident’s condition and be attuned to any change of condition.
- Monitor safety of residents and act to resolve problems and notify MD and team of care needs.
- Transporting residents to appointments, etc as needed.
Interactions
- Interacts with other health care professionals (MD, OT, PT, SW, etc.) to ensure resident health care needs are met.
- Transcribes MD orders, completes required forms.
- Contributes to care plan process with inter-disciplinary team on care needs of the residents.
- Acts as resident advocate.
- Educates residents on health care concerns/changes in care plan. Etc.
- Coaches/educates staff on health care issues.
- Attend educational workshops and in-services to maintain and update knowledge of changing procedures, methods, policies/procedures, laws and regulations relating to nursing practices.
Documentation/Admissions/Other
- Complete documentation in residents’ charts and provides continuity of care through this documentation.
- Administers medications and treatments per MD orders.
- Meets psychosocial needs of the residents by direct contacts and counseling.
- Assess and meet functional needs of residents with nurse assessments and planning.
- Facilitates medical appts. and assist to coordinate medical transports.
- Assigns duties as necessary to meet these.
- Documents residents’ healthcare status completes forms and documents the resident’s status.
- Completes all necessary agency documents and maintains the medical record.
- Provides continuity of care through documentation.
- Coordinates care with other nursing staff.
- Completes admissions and completes other job duties as assigned.
- Acts as resident advocate, educates residents and significant others and documents teaching.
Staff may be required to travel in a state vehicle to attend trainings, meetings etc
This position is designated as a supervisor over two other positions and will be responsible for supervisory and leadership duties.
MINIMUM QUALIFICATIONS:
Applicants must possess a valid, active Registered Nurse License issued by the State of Colorado (Department of Regulatory Agencies (DORA) OR any state participating in the Enhanced Nurse Licensure Compact (eNLC).
AND
Two (2) years of professional nursing experience
Substitutions:
Additional appropriate education will substitute for the experience on a year-for-year basis.

- Current/Valid and unrestricted US Driver’s License throughout employment
- Must complete reference check
- Must be at least 18 years of age
- Vaccines as required by the State of CO
- Must be available to work any shift, any day and/or any area, to meet agency needs, including weekends and holidays
- Must travel to remote work sites and must utilize a state vehicle, if available.
- If not, must utilize personal vehicle and submit for mileage reimbursement. Position is required to occasionally travel for trainings and/or resident transports.
- Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant’s prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.
