COLORADO STATE VETERANS COMMUNITY LIVING CENTER – HOMELAKE:
To ensure a safe, sanitary, well maintained, comfortable living and working environment for residents of a 60-bed skilled nursing home and 50-unit domiciliary, staff and visitors. The facility has embraced the Culture Change and Eden concepts. We strive to provide exemplary service to the residents by fulfilling daily and special requests that the residents may have. Exemplary care is given in cleaning and sanitizing and in keeping the resident rooms and their atmosphere clean, organized, homelike and safe. We need to provide the residents with the kind of service that they need and deserve.

Shift Information: Temporary Position. Hours to include days, evenings, nights, weekends and/or holidays (a CNA licensure is required for this position)
Duties required may include, but not limited to:
- ADL care to residents assigned including but not limited to i.e.: ambulation, ROM, dressing, toileting, bathing, transferring, positioning, lifting, skin care and/or assist with feeding.
- Care of resident belongings, room care: organized and floor areas free of clutter, moisture.
- Will follow proper infection control practices via the use of standard precautions.
- Communicates with others to ensure proper resident care and work completion.
- Maintains resident care and equipment such as cleaning oxygen concentrators, filling portable oxygen tank, maintain/disinfecting tubs, and showers, changing lift batteries etc.
- Report to charge nurse as to pertinent care issues: skin condition, falls, family concerns, and behaviors.
- Complete all documentation necessary for shift in the electronic heath record: including but not limited to, intake sheets, bath sheets, WEIGHTS, vital signs, bowel movement.
- Will occasionally transport resident, so will require a driver license.
- Other duties as assigned.
Education:
High School Diploma or GED
Special Qualifications:
Must possess valid CNA license from the Colorado Department of Regulatory Agencies upon entry into this position or have completed an accredited CNA program and obtain a CNA license within four months of completion.
Substitution:
A valid Certified Nurse Aide License (CNA) from the Colorado Department of Regulatory Agencies, or proof of completion of a board approved CNA program will substitute for the high school diploma or GED.
Preferred Qualifications:
- Experience in assisting residents during meals.
- Dependability
- Excellent Oral Communication Skills
- Problem Solving
- Participating as an Active Team Player
- Experience working in a long-term care setting.
- Experience in resident transfers, ambulation and recording intake and output measurements.
CONDITIONS OF EMPLOYMENT:
- Shift work required to include, days, evenings, nights, weekends, and/or Holidays.
- Must submit to a Full background check - Name check, TRAILS, Drug Screen and Fingerprinting
- Must possess a valid, unrestricted Colorado Driver's License
- Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
