Position Overview:
The Clinical Health Services (CHS) Immunization -License Vocational Nurse will provide vaccination services in Harris County through Texas Vaccines for Children (TVFC) and Adult Safety Network (ASN). The nurse will ensuring quality patient care under the clinical guidance/oversight of the CHS Advanced Practice Provider and Registered Nurse, the Immunization LVN will be responsible for performing vaccination tasks in accordance with TVFC & ASN regulations. The Licensed Vocational Nurse will be responsible for performing clinical tasks in accordance with with the federal, state, and local laws; rules, and regulations; and policies, procedures, and guidelines of Harris County Public Health (HCPH). This position supports the Harris County Public Health Department (HCPH), and reports to the Clinical Health Services (CHS) Site Supervisor. This position works in the community and on mobile units as needed.
Duties & Responsibilities:
- Administer age-appropriate immunizations, stays up to date with all CDC vaccine schedule changes, and assists the nursing team with always providing CDC-recommended vaccinations.
- Complete all documentation of patients records in the electronic medical record (EMR) system. Update patients shot records in the Texas Immunization Registry System referred to as Immtrac2.
- Review and screen patient shot records to make a clinical decision on immunizations needed.
- Support the vaccination coordinator's to ensure compliance with the Vaccines for Children's (VFC) & Adult Safety Network (ASN) Programs.
- Assists with the medical inventory of immunization supplies; Syringes, band-aids, alcohol swabs, and ensuring the team is appropriately stocked.
- Participates in performing quality assurance checks on vaccines arrivals and preparing vaccines for transport.
- Maintains knowledge of CDC recommended vaccines and administers age-appropriate immunizations as required. Acts as an alternate immunization coordinator for the Department of State Health Services Texas Vaccine for Children Program.

Education
- Associates Degree or diploma in Nursing
Licensure/Certification:
- Active license with no restriction as a Licensed Vocational Nurse in the State of Texas by the Texas Board of Vocational Nurse Examiners
- Basic Life Support (BLS) Certified
- Valid TX Driver's License
Experience:
- One (1) year of experience as a License Vocation Nurse in a clinical setting

- Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook) and navigating EMRs
- Excellent communication skills (verbal & written)
- Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment
- Knowledge of current CDC recommendations for immunizations
- Ability to organize material, prioritize job tasks, make reliable judgments, and draw conclusions from verbal discussions and written data
- Ability to draw conclusions from data, such as lab tests, to record this information accurately in patient’s record, and to notify the Clinical Manager or medical providers when deviations occur

Work Environment:
- This job operates in a professional clinic environment, community setting, and occupationally a medical mobile unit.
- This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Physical Demands:
- This role requires the ability to lift, carry, push, pull up to 50 pounds and bending & standing for periods at a time.
