6174 M/S Renal-4NAs related to the health care service setting, the RN Renal Coordinator is a nurse that provides timely intervention as appropriate to the renal patient's health condition and needs through collaboration with a multidisciplinary team including physicians.
QUALIFICATIONS
License /Certification
Required:
CA RN license
BLS
Preferred: National nursing certification related to nephrology
Education
Preferred: BSN, MSN, or currently enrolled in RN-BSN program.
Experience
Required: Minimum of one year RN experience providing direct nursing care to the end-stage renal disease patients, experience must be within the past three years.
Preferred: Two years or more of clinical experience as a RN caring for hemodialysis patients.
Knowledge/Skills/Abilities
Bilingual in Spanish is preferred.
Department Specific Requirements
For Chronic Renal Care Coordinator: must have current clinical experience as a RN caring for chronic hemodialysis patients, experience within the past three years.
JOB RESPONSIBILITIES
Essential
Gathers information and communicates to patients and patient's family members/significant others in culturally and linguistically appropriate terms.
Keeps clinical knowledge current related to End-Stage Renal Disease and healthcare management of this patient population.
Is a positive representative of the organization in speech and behavior, promoting collaborative, interdisciplinary teamwork with others such as patients, family members, peers, other departments, and physicians.
The nurse's practice is guided by the Code for Nurses. Decisions and actions on behalf of patients are determined in an ethical manner. Maintains patient confidentiality within legal and regulatory parameters. Acts as a patient advocate and assists patients in developing skills so they can advocate for themselves. Delivers care in a nonjudgmental and nondiscriminatory manner that preserves patient autonomy, dignity and rights.

Acute Renal Access Coordinator also responsible for:
Develops and maintains strong partnerships with nephrologists, vascular surgeons, and other physicians involved in access insertion and management.
Assists the physician in renal access management per critical pathways through tracking, facilitating and maintenance of the patient's renal access.
Identifies new dialysis initiates.
Provides education to new initiates, dialysis patients related to infection prevention home activities.
Provides education to new initiates, dialysis patients, related to temporary tunneled or non-tunneled catheters and as related to long-term placement of fistulas (national early access initiative).
Supports the collaborative process to promote cost effective quality outcomes for the dialysis population through the transition of the renal patients through the continuum of care.
Facilitates transition of patient toward safe, timely discharge.
Timely referrals are made for services, both as needed at Kaweah Health and dialysis clinic placement.
Provides timely communication to manager/director of barriers to access placement if placement is urgent and normal process is not effective. Initiates action steps to achieve resolution and provides manager/director with possible solution(s) to resolve concern. Informs Medical Director as appropriate.
Provides timely communication to manager/director of impending area(s) of non-compliance before non-compliance occurs. Has already sought resolution and provides manager/director with possible solution(s) to resolve concern. Informs Medical Director as appropriate.
Assures patient access site quality indicators are met for clinic through monitoring, documentation, and education. Assures reports are completed and submitted on time. May delegate some activities to appropriate personnel.
Monitors quality indicators as assigned by manager/director.
Provides a monthly report at the renal services Quality Assessment and Performance Improvement meeting.
Able to intermittently provide direct patient care to hemodialysis patients as supported by the RN job description. Maintains competencies to support this essential function. This function is requested only as emergent need arises and as directed by the nurse manager or director.
Serves as educational resource for dialysis staff regarding vascular access.

Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area. Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs.
