7290 Home Health AgencyEvaluates and processes all new incoming referrals assuring patients meet criteria and have a current physician overseeing care. Obtains authorization for care to be provided, ensures smooth transition from Hospital discharge to arrival at home.

JOB RESPONSIBILITIES
Essential
Responds to inquiries about services within one working day. Provides program information in a professional and thorough manner.
Develops a positive working relationship with all physicians and staff involved in the referral and patient transfer process.
Upon receiving in-house patient referrals, performs a site visit within one working day (M-F). Reviews medical information, assesses patient based on relevant program admission criteria, provides agency choice and documents choice and assessment findings on appropriate forms.
Discusses program and patient needs with patient, family, physicians and staff to obtain a complete patient profile. Involves all parties, but particularly the patient and/or family in the decision-making process regarding transfer.
Establishes the availability of funds for the services required. Communicates with patient and appropriate administrative and patient accounting personnel throughout the process.
Communicates with patient, family, physician, and referral source regarding admission process and date. Provides information regarding other options if admission is not possible.
Communicates with unit staff, patient and case management concerning special wound supply needs/equipment of patient to be admitted to home health.
Maintains current authorizations for all insurance sources.
Maintains monthly statistics through referral log.
Assures all intake information is complete and accurate including patient information (phone number and address) physician information (name, address, phone number, fax number) and insurance information.
Inputs all necessary data into the intake system accurately and timely.
Responsible for securing and documenting all insurance authorizations and reauthorizations on assigned cases working closely with agency case managers to assure all visits are authorized.
Manages all outside referrals and accepts referrals based on policy assuring that outside hospital referrals have a local physician that has agreed to follow the patient.
Obtains physician orders on all new referrals and resumes.
When orders not provided at discharge, if needed, obtains appropriate resume orders for patients currently on service within 24 hours of discharge.
Performs a review on all completed admissions within five business days of receipt checking all areas outlined in the admission review process to assure complete and accurate information.
Participates in daily hospital rounds.
Conducts face-to-face encounters with patients being discharged from Kaweah Health Hospital and admitted to Home Health for the purpose of explaining services and obtaining correct information.
Calls all patients being referred by a physician's office to determine whether patient meets home health guidelines for admission prior to accepting referral.
Documents in Elmer notes all information pertinent to patient discharge and transfer to home health.
Notifies patient's PCP of home health referral prior to accepting referral to assure PCP is willing to follow and sign for home health services.
Assists in the management of patients being discharged to areas outside of our service area by contacting and arranging services with an agency in the area patient will be residing.

HOSPICE
Responds immediately to inquiries and referrals from physicians, healthcare providers, patients, families and friends. Coordinates consults/date/time with discharge planners, physician's offices, patients, families and caregivers within three (3) hours of receiving referral. Assesses appropriateness of the patient for services; collaborates with the KDH physician, manager or care team as needed. Ensures all consults/admissions are completed the same day received unless otherwise requested. Demonstrates expertise and understanding of criteria for admission. Ensures compliance with State, Federal, and other accrediting agency regulations. Educates about Hospice services and offers information regarding other community resources as needed. Obtains insurance or payer information from the referral source; clarifies Medicare certification period; coordinates process, when needed, for face to face visit by physician. Accurately codes diagnoses on the referral form. Collaborates with Team Managers about assignment of team members. Documents timely and accurately: electronic medical record and paperwork. Obtains verbal orders from physicians, as needed, to expedite completion of referral process. Monitors readiness of completed charts for the admissions nurse. Leaves report for the admissions nurse. Initiates follow up for outstanding intake/referral process needs. Initiates pre-billing start of care audits and forwards timely to Team Secretary. Ability to handle sensitive issues. Delivers optimal professional service to our external and internal customers at all times. Participates in outreach as requested. Participates in on-call activities. May perform patient care.
