Overview of Position:
The LPN functions under the supervision and guidance of the RN Care Manager and RN Manager of Inpatient Care Management. The LPN will participate in the multi-disciplinary process of providing care and treatment to patients. They provide clinical support and technical assistance for Care Management and our patients. The LPN is responsible for functioning within the scope of practice dictated by the authority of the Ohio Board of Nursing for the Licensed Practical Nurse. The individual must be able to manage a demanding workload with accuracy. The position requires excellent customer service skills, phone skills, clinical knowledge along with communication skills.
ESSENTIAL DUTIES
1. Post-Acute Service Knowledge: Establish and maintains a knowledge base of skilled nursing services, home health care, DME providers, hospice and palliative care services along with other post-acute care services.
2. Post-Acute Care Quality Data monitoring: Participates in data collection, and maintenance of scorecard. Assures documentation is complete and accurate in the EPIC system.
3. Post discharge follow-up process: Ensures that transitions of care are completed for his/her assigned patients, and that patient has the appropriate resources to improve care outcomes and prevent readmissions. Explains system wide care management to their patients and ensures that they know how to reach them when they have questions or concerns. Considers the patient’s goals and preferences when developing the patient’s system wide care plan.
4. Communicates effectively (verbal, written and telephonic) with patients, families, and other care givers, with tact, diplomacy, sensitivity and professionalism.
5. Safety: Contributes to modifications in care management protocols to continuously improve quality of care, patient satisfaction, patient safety and appropriate use of resources.
6. Facilitate BPCI initiatives throughout the continuum of care. Facilitate tracking of BPCI goals that consist of the following: Next site of care / Optimizing Utilization; SNF selection / Utilize Performing Providers; SNF length of stay (LOS)/ shortening LOS when clinically appropriate; and Readmissions – Avoid Preventable Readmissions.
7. Patient Education: Provides patient education related to preventive health topics such as tobacco cessation, nutrition, physical activity, immunizations, health screenings, diabetic education etc. Provides education related to medication adherence, appointment compliance and general health.
8. Readmission Prevention strategies with Post-Acute Care Agencies: Assist with readmission root cause analysis and action plan development. Some examples include ensuring that appointments are set up, patient has transportation, medication authorizations are completed, and referrals are scheduled when appropriate.
QUALIFICATIONS
1. LPN licensure required in the state of Ohio.
2. 3 years of nursing experience in a hospital, primary care office, nursing home or home health setting is required.
3. Current CPR certification or ability to obtain within 30 days of employment.
4. Ability to perform data analysis and to utilize computer systems to record and communicate information to other services.
5. Excellent communication, verbal and organizational skills to order to facilitate the post-acute care management process.
