JOB SUMMARY
The Home Health/Hospice Aide Certified Nursing Assistant provides personal care, home hygiene services and other supportive services to persons at their home or residence. This individual is responsible for accurate record keeping, documentation of the routine and delegate duties completed and notification to the Registered Nurse of any change in status. The Home Health/Hospice Aide CNA provides services and cares in accordance with a comprehensive patient care plan and under the supervision of a RN, physical therapist or occupational therapist.
Schedule: 8 hour day shifts and weekend rotation of once per month
ESSENTIAL JOB FUNCTIONS
1. Provides for safety, cares and assists patients through the plan of care/care plan as directed by the Registered Nurse (RN) to ensure patients’ needs are met.
2. Communicates with patients and co-workers information as necessary to ensure desired patient goals.
3. Collaborates with direct patient care team members to ensure patient care services are adjusted and completed with the patient’s changing care needs.
4. Provides cares and services within the scope of practice as a Certified Nursing Assistant (CNA) under the direct supervision of RN to ensure work assigned is completed in a timely and accurate manner.
5. Operates multiple software systems required for patient care and business operations to appropriately and accurately document patients cares and communicate patient information to the health care team in a timely manner.
6. Drives to various locations to complete the functions of the role.
7. Works independently in various homes, facilities and patient settings to provide care to Home Health Care Hospice patients.
8. Maintains department supplies and equipment needed for patient care to ensure appropriate items are available and within acceptable infection control guidelines.
9. Regular attendance is required in order to carry out the essential functions of the position.
10. Reviews and meets ongoing competency requirements of the role to maintain the skills, knowledge and abilities to perform, within scope, role specific functions.

JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.

Preferred/Optional: Experience in hospital or nursing home.
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: Certified Nursing Assistant (CNA) certification awarded by the State of Wisconsin. Listed on the State of Wisconsin Registry as eligible to be employed at a state and federally –certified Home Health and Hospice agency. Basic Live Support (BLS) certification provided by the American Heart Association within three months of hire. Valid State of Wisconsin Driver’s license with acceptable driving record.
Preferred/Optional: Certification in Hospice and Palliative Care awarded by the National Hospice and Palliative Care organization (NHPCO).
