JOB SUMMARY
The Certified Nursing Assistant (CNA) -Long Term Care works under the direction of a Registered Nurse and in accordance to the patient/family care plan to collect patient information and provide direct patient care contributing significantly to the achievement of patient and family outcomes. As a member of the direct care team, the CNA is responsible and accountable for complete and accurate collection and recording of data, completion of routine and delegated direct patient care activities and communicating and collaborating with the Registered Nurse to prioritize workload.
Hours: Mon-Fri 10:30pm-6:30am, every other weekend and holiday rotation
ESSENTIAL JOB FUNCTIONS
1. Provides personal care, bathing, and grooming needs delegated by licensed staff and in accordance with accepted standards of practice.
2. Maintains nutritional needs including fluids and snacks based on resident’s individual needs.
3. Completes all restorative and ROM programs.
4. Participates in the hourly rounding program to ensure safety to the residents.
5. Provides excellence for the resident’s by displaying characteristics of care, compassion and empathy.
6. Reports and communicates clear and concise reports with other team members.
7. Reports changes in resident status to RN, and other team members.
8. Participates in care planning and behavioral meetings.
9. Communicates well with family and resident regarding care of resident.
10. Documents completely, accurately and timely according to department guidelines.
11. Safeguards patient confidentiality in verbal, written and computer documentation.
12. Knows and follows policy and procedures for safety precautions.
13. Utilizes sound principles of body mechanics.
14. Uses standard precautions: uses personal protective equipment (PPE); uses appropriate linen and waste disposal.
15. Ensures resident safety through accurate use of safety devices, participation in post fall huddles and follow through of Care Plan.
16. Regular attendance is required in order to carry out the essential functions of the position.
17. Reviews and meets ongoing competency requirements of the role to maintain the skills, knowledge and abilities to perform, within scope, role specific functions.
ADDITIONAL DUTIES
1. Protects organizations value by keeping resident information confidential.
2. Other duties as assigned.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.

EXPERIENCE
Minimum Required: None
Preferred/Optional: Experience working as a Certified Nursing Assistant (CNA) in a long term care setting.
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: Must have and maintain a Certified Nursing Assistant (CNA) certification awarded by the State of Wisconsin. Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.
Preferred/Optional: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
