STAFF RN-CASE MANAGER - WOUND CARE CENTER

JOB RESPONSIBILITIES:

Assessment:

Initiate a nursing assessment of patient’s condition upon admission.
Assess and interpret reports communicating findings with the assigned provider.
Recognize and assess initial alterations to the patient’s body systems and communicate with clinical team.
Organize and record assessment data for accuracy, completeness and confidentiality.
Assess, stage and measure wounds using photographic technology accurately and in accordance with policy and procedure.
Identify patient care situations that require intervention, implement nursing actions and evaluate responses.
Work closely with assigned provider to assess compliance to clinical pathway and communicate missing components effectively.
Identify educational and discharge needs to patients and their families and implement a teaching plan of care.

Planning:

Identify patient’s present and potential problems from the assessment and collaborate with provider to develop a plan of care incorporating patient’s health status.
Formulate desired outcomes with provider and ensure they are mutually agreed upon by the patient, family (when appropriate) and the clinical team.
Identify home health care needs, evaluate support systems and identify patient educational needs from admission to discharge.
Continue to assess patient’s plan of care to determine if changes are necessary.
Coordinate activities with other disciplines (rehab, nutrition, diabetic education, home health, DME, nursing homes, and other ancillary services).

Implementation:

Actively participate in implementing treatment plans, ensuring appropriate changes are communicated effectively to both the clinical team and patients.
Implement wound care and HBO protocols when clinically indicated.
Assist providers with advanced wound therapies.
Document timely within the electronic medical record to ensure charts, plans of care and superbills are complete and accurate.

Evaluation:

Evaluate achievement or lack of achievement of desired outcomes.
Collaborate with providers and the clinical team to ensure plans of care are appropriate, revising as necessary, and continually evaluating until desired outcomes are realized.
Complete 4-week reviews in a timely manner and ensure all recommendations to the plan are activated in the provider orders.

General duties include but are not limited to the following:

Functions as case manager for specific patient population.
Coordinates office visits, pre-procedure work-ups and patient education with clinical team.
Fosters continuity of care between the WHC, caretakers and family members.

Clearly communicates and continuously supports the Mission and Values of JMH.

Performs additional duties not addressed here as assigned by the Wound Care Supervisor in conjunction with the Program Director.

Conducts all activities in compliance with applicable laws, regulations, standards and JMH policies and procedures including Blood and Body Substance Precautions.

EDUCATION, EXPERIENCE AND TRAINING:

Must be licensed in state of Indiana as a Registered Nurse; CPR certification and minimum of 2 years Medical/Surgical experience required. Wound care experience preferred. Current BLS or CPR for Healthcare Providers certification required.

SCHEDULE: Monday through Friday from 8:00 a.m.-4:30 p.m.

Monday through Friday from 8:00am-4:30pm
80 hours per two-week pay period.
