Job Summary

Under the supervision of the Operations Leader, Clinical Resource Team Leader and Hospice RN's the licensed practical nurse performs direct and indirect patient care activities and related services necessary for the care and comfort of the Hospice patient. Work activity will occur in patient's home, family's home, or nursing home. Works closely with all members of the interdisciplinary team, physicians and area agencies.

Job Responsibilities

Under the supervision of the Operations Leader and/or Clinical Resource Team Leader, the LPN will perform as assigned:
- The counsel and health teaching of the patient and caregiver.
- Procedures as an extension of the therapeutic services.
- Ambulation and exercise of the patient.
Reporting changes in the patient's condition and needs to the Operations Leader, Clinical Resource Team Leader or primary nurse.
Responsible to the client/patient/family in the home for services within the scope of the practical nurse.
Completion of appropriate records.
Maintain regular communication with the primary nurse to review care.
Attend Hospice interdisciplinary team meetings and other patient conferences as deemed necessary by the Director and/or the Operations Leader.
Adhere to the philosophy, policies and procedures of Hospice.
Provide appropriate support at time of death.
Participate in FHN in-service programs, agency and community programs as requested to promote professional growth and understanding of Hospice care.
Protects the confidentiality of patient/family information at all times.
Performs other related duties and special projects.

Displays competency for the Licensed Practical Nurse Hospice role as evidenced in the annual performance appraisal following the FHN PRIDE Standards.
Other duties may be assigned.

Qualifications

Requirements

Education: Graduate of an accredited school of nursing.

Required Experience: Two years of clinical experience in an institution of acute patient care, a licensed nursing home, or licensed home health agency.

Licensure/Certification/Registration:

BLS Certification
Current licensure as an LPN in the state of Illinois
Valid driver’s license and proof of vehicle insurance

Early Childhood (1 year and < 5 years old)
Late Childhood (5 years and < 13 years old)
Adolescent (13 and < 17 years old)
Young Adult (17 to < 30 years old)
Middle Adult (30 years to < 60 years old)
Older Adult (> 60 years old)
Serves patients in areas other than direct patient care
