Job Summary
LPN-Ambulatory Office- Pediatrics
Location: FHN Family Healthcare-Burchard Hills, Freeport, IL
Position: Full-time
Primary Hours: 8am-5pm
This position offers a $3,000 commitment award with a 3 year commitment to FHN for a Licensed Practical Nurse.
Delivers nursing care to patients and their families utilizing the nursing process in conjunction with evidence based practice, using a holistic approach. Displays critical thinking, a commitment to excellent customer service, and technical and interpersonal skills to achieve optimal patient outcomes. Encompasses a multitude of tasks that contribute to the efficient operation of the healthcare practice. Incorporates a positive team approach to serve as a liaison between physician office and other members of the healthcare team. Key public relations position which will involve patient's compliments and concerns.
Job Responsibilities
- Focuses communications in patient/customer care, business operations, and/or topics to create a positive work environment.
- Participates in the assessment of the patient by observing, collecting, documenting and reporting:
- Objective and subjective data in an accurate and timely manner.
- The condition and/or change in condition of the patient.
- Signs and symptoms of deviation from normal health status.
- Using telecommunication, is able to collect patient information and communicate information to appropriate staff/provider, implementing aspects of the plan of care as delegated.
- Performs and maintains competency for all point of care clinical testing required for that department, including collection and processing of specimens as applicable to job role.
- Works collaboratively with provider in assisting with procedures and treatments, obtaining appropriate consent(s) as needed.
- Prepares and administers medications and treatments as ordered by the provider within scope of practice following FHN policies and procedures and documents the same.
- Participates in healthcare education and counseling to promote, attain, and maintain the the optimum health level of patients.
- Monitors, maintains (checking for out dates), and orders medications and supplies adequate to deliver patient care as applicable.
- Communicates diagnostic test results and provide education as directed to patients upon provider orders and documents the same.
- Ensures appropriate techniques are utilized in cleaning equipment, work area and exam rooms. Uses proper processes for reporting malfunction of equipment and takes out of service as needed.
- Documents (utilizing approved abbreviations) all nursing interventions and patient responses to care utilizing established guidelines and protocols.
- Schedules testing for patients (labs, referrals, appointments) providing appropriate patient education ensuring completion of appropriate paperwork, including but not limited to, waivers, pre certifications, and referrals, and disability forms as applicable.
- Continually evaluates office practice to identify opportunities for improving safety and quality of care by bringing forth ideas and suggestions.
- Maintains neat appearance of work area, patient care area, and waiting area conducting light housekeeping as needed.
- Provides age/culturally appropriate, evidence based care.
- Displays a competency for the Licensed Practical Nurse as evidenced in the annual performance appraisal utilizing the FHN PRIDE Standards.
- Provides superior, individualized customer service for patients, families and other visitors.
- Serves as a patient advocate, proactively working to identify and resolve patient concerns while practicing within FHN policies
- and procedures.
- Understand and able to perform functions related to computer programs, telecommunication equipment and other technology.
- Participates in activities to support FHN's mission, goals and strategic plan.
- Serves as a mentor or preceptor to coworkers displaying a positive nursing image and promoting a positive educational experience.
- Positively works to identify and resolve patient concerns and refers to appropriate party if necessary.
- Performs any and/or all duties within scope of practice.
- Other duties may be assigned.
Qualifications
Requirements
Education: Graduate of an accredited nursing program; two to five years related experience and/or training; equivalent combination of education and experience. Knowledge of CPT and ICD-9 coding, as well as knowledge of third party reimbursement regulations preferred.
Required Experience: Must have ability to handle multiple tasks in a fast paced work environment. Must be flexible to the scheduling of work hours to meet the needs of the FHN, which may include cross departmental functions.
Licensure/Certification/Registration:
- Licensed Practical Nurse currently licensed to practice in the state of Illinois.
- BLS certification required
