What it takes to become a MDS Coordinator (LPN) with Care Initiatives:

Valid Licensed Practical Nurse (LPN) license in good standing. License must be valid within the state of Iowa, or ability to activate.

·Current, valid CPR certification.

·MDS credentialed or willingness to obtain.

·Familiarity with MDS 3.0/RAI Manual and work experience in long-term care and gerontological nursing are highly preferred.
