JOB SUMMARY
The Certified Nurse Assistant (CNA)-CBRF provides any combination of the following duties–direct resident care and assistance with activities of daily living (ADL), and other unit activities as delegated under the supervision of a Registered Nurses (RN). This individual assist in the maintenance of a safe and clean environment, recreational and diversional resident activities, therapeutic dialogue and teaching. Documentation and reporting are performed to promote resident and family wellness.
ESSENTIAL JOB FUNCTIONS
1. Collaborates with direct resident care team members ensure resident care services are adjusted and completed with the residents changing care needs.
2. Collaborates with the RN to establish mutual care priorities on a shift-by-shift basis.
3. Communicates meaningful pertinent information to appropriate individuals to assist in accurate care for the resident.
4. Assists the resident and family to complete activities of daily living, anticipates the patient’s needs and modifies the approach based on the plan of care, resident coordination, resident age and development and resident request(s) to allow growth and development of the resident and family functions.
5. Performs general ADL, hygiene, assist with ambulation and/or exercises and dietary assistance in the dining room/resident’s room and activities.
6. Determines and records vital signs to ensure accurate information is available as needed.
7. Monitors appetite, intake, and output, obtains specimens to report observations to the nurse.
8. Initiates resident contacts to determine current overall status and monitors progress throughout assigned shift.
9. Monitors assigned residents continually for change in condition.
10. Performs whole unit duties at assigned times to ensure work assigned is completed in a timely manner.
11. Performs some specialized procedures within all regulatory requirements and within scope of practice.
12. Acts as a liaison for family and resident providing support and educational services from time of admission and throughout stay.
13. Completes delegated admission and discharge activities using appropriate forms and protocol.
14. Ensures that resident’s rights are respected, and infringements reported to appropriate personnel to ensure resolution in a timely manner.
15. Conducts and communicates other resident interviews to elicit significant information.
16. Assists with light housekeeping/laundry to straighten resident quarters on a daily basis as needed and ensures toilet supplies are in resident’s room.
17. Makes appointments and arranges transportation to medical appointments to ensure resident arrives at destination in a timely manner.
18. Accompanies resident as needed to medical appointments ensuring residents needs are met.
19. Practices and maintains universal precautions while performing direct care duties.
20. Supports the professional nurse by promptly and accurately completing assigned tasks, including the necessary written documentation and oral reporting. 21. Directs visitors ensuring exceptional services are provided.
22. Participates in educational opportunities provided by CBRF and MMC-BD, maintaining 15 hours continuing education annually per DHS 83 regulations.
23. Understands and participates in disaster drill (fire, tornado, elopement) following the emergency plan of action.
24. Answers phones to ensure calls are directed to appropriate individuals.
25. Regular attendance is required in order to carry out the essential functions of the position.
26. Reviews and meets ongoing competency requirements of the role to maintain the skills, knowledge and abilities to perform, within scope, role specific functions.

JOB QUALIFICATIONS
EDUCATION
For positions, requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: CBRF required training per DHS 83.20 approved training:
- Fire Safety- CBRF specific- to be completed within 90 days after starting employment
- First Aide and Choking- CBRF Specific- to be completed within 90 days after starting employment
- Exception: Emergency Medical Technicians
- Medication Administration and Management- Any employee who manages/administer and assist residents with prescribed or over the counter medications need to complete the training prior to assuming job duty and complete post training competency with RN with documentation.
- Exception: CNA who has completed a medication aide training and is in good standing on the Wisconsin Nurse Aide Registry
- Student Nurse currently enrolled in nursing program and successfully completed
- medication administration course with documentation of completion
- Dietary Training – specific topics include nutritional needs, menu planning, food preparations and food sanitation within 90 days of hire
- Assessment of Resident and Individual Service Plan (ISP) within 90 days of hire
Preferred/Optional: Completion of DHS approved courses in Fire Safety, first Aid & Choking, Medication Administration and standard precautions at time of hire.
EXPERIENCE
Minimum Required: Customer service and communication skills. Basic knowledge and use of computers.
Preferred/Optional: Experience in a hospital, health care center, long term care, home health agency, or residential treatment facility or other health treatment program.

JOB SUMMARY
The Resident Assistant is responsible for the basic resident care provision and promotion of non-acute quality care to the population of the community based residential facility (CBRF) as outlined in Individual Service Plan designed to meet the needs of our residents, and in a manner, that follows accepted standards of practices, policies, procedures, and State Regulations. The Resident Assistant will follow all policies/procedures and nursing standards under the supervision of the RN, LPN, and CNA.

ESSENTIAL JOB FUNCTIONS
1. Accepts and completes personal hygiene and daily living needs in accordance to Resident Assistant registration and DHS-83 regulations.
2. Assists residents with medication administration including accuracy and documentation per DHS-83 and Resident Assistant medication training. 3. Properly obtains vital signs information including pulse, blood pressure, respirations, temperature, and pulse oximeter level.
4. Assists with planning and implementation of person centered and purposeful activities.
5. Provides nutritious snacks and adequate fluids to meet individual resident needs and maintain appropriate weight, report any changes to CNA, Lead, Manager, or Director.
6. Involves residents in cooking based and purposeful activity based on interests and capabilities.
7. Provides input regarding planning and preparing meals for special events.
8. Provides feeding assistance when needed. Maintains sanitary condition within the kitchen and practices.
9. Communicates with other staff regarding their responsibility in the preparation and serving of meals.
10. Professional communication skills with interdisciplinary team, family and resident regarding care of resident audibly, verbally and utilizing computer and written procedures.
11. Observes residents for changes in behaviors, reactions, or change in health symptoms and report to CNA, Manager, and Manager on Call, or Director.
12. Documents cares and actions by completing all necessary charting requirements.
13. Utilizes a variety of technological devices and software effectively: computers, emails and phone etiquette.
14. Completes housekeeping, laundry and cooking duties as assigned. Maintaining a safe, functioning environment by keeping areas clean, organization and equipment in good repair.
15. Ensures residents safety by promoting continuous improvement for quality and safety of residents and facility. Demonstration of accurate use of safety devices, participates in post fall huddles, and follows through on Individual Service Plan, utilizing recourses efficiently.
16. Participates in hourly rounding program to ensure safety of the residents.
17. Assists Senior Services leadership with identifying areas of improvement and implementation of change to provide high quality of care.
18. Participates in educational opportunities provided by CBRF and MMC-BD, maintaining 15 hours continuing education annually per DHS 83 regulations.
19. Understands and participates in disaster drill (fire, tornado, elopement) following the emergency plan of action.
