Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary

Serves as a nurse team specialist in the coordination of care for adults and children with medical complexity as appropriate intensity to accommodate each family’s strengths and needs.
Applies nursing scope of practice, and clinical experience and knowledge in the care of patients with chronic health conditions and disabilities.

Essential Functions and Responsibilities

•   Models Eskenazi Health's mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County.

•   Participates in the identification of eligible patients within the practice, to build a full registry of assigned patients to serve.
•   Leads the practice in obtaining consent from patients/families, in summarizing medical records, in performing a comprehensive assessment, developing, sharing, and implementing a shared plan of care, with ongoing facilitation of each patient/family’s achievement of their named goals.
•   Works closely within primary care team, including clinicians, nursing, and other team members, to facilitate comprehensive service delivery to the assigned registry of patients, both within and beyond the primary care setting.
•   Seeks both family strengths and barriers to care, including social determinants of health, to assist in the formulation of effective plans of care and achievement of negotiated actions.
•   Assist patients and families with the identification of problems and obstacles to each patient/child’s progressive development towards his/her future goals and develop strategies to mobilize community resources utilizing both formal and informal support.
•   Participates in the identification of clinical or operational performance improvement opportunities and in performance improvement activities using evidence-based practice.
•   Utilizes the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of patient care. Works in conjunction with other care providers.
•   Utilizes established protocols to manage phone, walk-in triage of patients to identify level of care needed, triage priority, and implementation of an appropriate plan of action.
•   Provides, ensures the provision of patient care which reflects nursing intervention and considers patient response to care provided, patient needs, problems, capabilities, limitations, and progress toward goals. Alters activities/behaviors ensure adequate care appropriate to the age of patients served (i.e., pediatric, adolescent, adult, elderly)
•   Documents assessments, and all other nursing activities (including medication administration) legally, legibly, concisely, descriptively per policy/procedure. Monitors follow-up of tests and lab studies done and notifies provider.
•   Teaches patient/significant other appropriate health information and shares written informational material if available. Documentation includes evidence of appropriate patient/significant other teaching and understanding of these instructions is documented in the medical record.

Job Requirements

•   Minimum Experience: 2 or more years of clinical experience required.
•   Experience with pediatric and adult patients with complex medical conditions preferred.
•   Experience as care coordinator/case manager preferred.
•   Current Indiana license as a Registered Nurse
•   CPR certification (American Heart Association Healthcare Provider preferred)

Knowledge, Skills & Abilities

•   Competence/accountability in administering medications, treatments, performing tests.
•   Exhibits effective communication skills to include effective listening and interviewing techniques.
•   Demonstrates knowledge of diseases/symptoms, usual treatments/ medications /interventions, current clinical concepts
•   Demonstrates ability to function as member of the healthcare team and to delegate responsibilities/duties appropriately.
•   Able to follow oral/written instructions and record/interpret information accurately.
•   Demonstrates basic computer skills.
•   Demonstrates critical thinking skills and can give rationale.
