Hospice Triage Visit RN

Hospice Triage/Visit RN | The Registered Nurse plans, organizes, directs and provides hospice care to meet the identified needs of patients/families. This position will provide care to patients after normal business hours either by phone triage or to go on visits when applicable.

Hours: Friday at 5:00pm - Monday at 8:00am | You would be on call during these hours and this position is considered full-time and is benefit eligible.

Territory: Our office is based in Roseville, MN and our service area covers the 13 County Twin Cities Metro area. We are looking for someone who is willing to travel throughout Interim's service area based on the client’s needs.

Minimum Requirements:

Licensed to practice as a Registered Nurse in the state of MN. License is active and in good standing. Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS. One (1) year of hospice care experience preferred. Able to travel within geographic area serviced by office. Valid drivers license, and proof of auto insurance is required. CPR Certification required

Some functions of what you would do as an On Call Triage RN

Completes ongoing assessments of the patient’s physical status and the psychosocial status of the patient/family. Initiates and develops a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family and the attending physician. Makes necessary revisions to the IDG plan of care as the patient’s/family’s status and needs change. Evaluates and coordinates the need for services provided to the patient/family in accordance with the IDG plan of care. Administers medications and treatments as prescribed by the physician and in accordance with nursing standards of practice.
