Hospice Registered Nurse (RN) Case Manager: Enjoy independence, professional autonomy, and flexibility in the field to visit patients one on one in their own homes. Hours: Full-time - Days and On Call Rotation Territory: The RN will have a general territory within our service area where they will provide in-home Hospice care to our patients. The territory for this position would primarily be in the South West section of the Twin Cities Metro area (i.e. Cities would include those that are South of Highway 394 and West of 35W). A few must-haves for this position: Licensed to practice as an RN in the state of MN. License is active and in good standing.Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate. One (1) year of hospice care experience preferred.Able to travel within geographic area serviced by office. Valid drivers license, and proof of auto insurance is required.CPR Certification Some functions of what you would do as a RN Case Manager: Completes an initial/comprehensive assessment of the patient/family to determine hospice needs which includes a history of current and previous illness(es), as well as a physical assessment of the patient, with emphasis on the management of pain and other symptoms.Completes ongoing assessments of the patient’s physical status and the psychosocial status of the patient/family.Initiates and develops a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family and the attending physician.Makes necessary revisions to the IDG plan of care as the patient’s/family’s status and needs change.
