What You Will Do as a Hospice RN Field Nurse

As a Hospice RN Field Nurse, you will provide support to patients and families to ensure a safe and comfortable environment.

Some of your primary responsibilities will include:

Completing an initial assessment of patient and family to determine hospice care needs. Providing a complete physical assessment and history of current and previous illness
Developing a care plan, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative actions
Re-evaluating and revising patient care needs as needed
Administering medications and treatment safely and as prescribed by the physician
Counseling and educating patients and families in meeting nursing and related needs
Providing health care instructions to the patient as appropriate per assessment and plan of care
Participating in on-calls

Schedule

This is a PRN position that is likely to become Full Time as the hospice division grows.

Education, Licensure & Certification Requirements

Graduate of an accredited school of nursing
Active RN license
One (1) to two (2) years of recent acute care experience
Bachelor’s degree with one (1) year of hospice/home health experience preferred
