Job Title: Full-Time Hospice RN Case Manager -

$3000 sign-on bonus

Location: Wyoming Valley, PA

Servicing - Lackawanna & Luzerne PA Counties

Position Overview: As a Full-Time Hospice RN Case Manager at Serenity Hospice PA, you will play a vital role in delivering comprehensive and compassionate care to our patients. Working in our supportive and staff-oriented environment, you will have manageable caseloads, allowing you to provide personalized attention and make a meaningful impact on the lives of patients and their families during their end-of-life journey.

Responsibilities:

Collaborate with the interdisciplinary team to develop and implement individualized care plans for hospice patients
Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs
Coordinate and manage patient care, ensuring the highest level of comfort and quality of life
Regularly assess and reassess patients' conditions, adjusting care plans as necessary
Administer prescribed medications and treatments in accordance with physician orders
Educate patients and their families about disease progression, symptom management, and end-of-life care options
Provide emotional support and counseling to patients and families, fostering a compassionate and understanding environment
Maintain accurate and timely documentation of patient care activities
Adhere to all applicable laws, regulations, and company policies to ensure compliance

Qualifications:

Valid Registered Nurse (RN) license in the state of Pennsylvania
Minimum of one year of hospice experience
Proficiency in performing comprehensive physical assessments and managing pain and symptoms
Strong interpersonal and communication skills
Ability to work collaboratively as part of a team and autonomously
Compassionate and empathetic approach to patient care
Excellent organizational and time management abilities
Preferred proficiency in electronic medical record (EMR) systems
