Responsibilities

Deliver Patient-Centered Home Care: Provide high-quality nursing care, perform assessments, and develop and update individualized home health care plans in collaboration with patients, caregivers, and physicians.
Ensure Accurate Documentation and Compliance: Maintain detailed clinical records, ensure compliance with regulatory standards, and participate in quality assurance activities to uphold best practices and patient safety.
Collaborate and Supervise: Coordinate with interdisciplinary teams, communicate patient needs, supervise home health aides and LPN staff, and participate in education and training efforts.

Qualifications

Education:

Graduate of State Board approved program for Registered Nurses

Experience:

One year nursing experience caring for similar patient population as to be assigned.

License(s)/Certifcation(s):

Valid license as Registered Nurse in state where providing care.
Valid licensed driver with automible insurance in accordance with state(s) and/or organizational requirments.
Mandatory Reporter: Child + Dependent Adult Abuse
Person Centered Care (PCC) course completion within first 12 months of hire + annual completion of competency validation activities.
CPR: Maintian a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certifcation
