Nursing Supervisor, Community Based (Internal HSPH applicants only)

The Long-Term Services and Supports area of Health and Human Services is seeking a Community Based Nursing Supervisor for the Initial Assessment area to work with our south-west metro team. This position will lead a team completing initial assessments for individuals living in the community. This position will support and manage an interdisciplinary team of public health nurses, social workers, and case management assistants. The team will complete comprehensive initial assessments, determine eligibility, develop support plans, and authorize services for seniors and populations with disabilities which may include brain injury, mental health disorders, and complex medical, intellectual, and physical disabilities. The federal and state programs that the team administers and provides access to include the following: Alternative Care (AC); Elderly Waiver (EW); Community Access for Disability Inclusion (CADI); Brain Injury (BI), Community Alternative Care (CAC), Developmental Disabilities (DD), and/or Community First Services and Supports (CFSS/formerly Personal Care Assistance (PCA)).

Location and hours:

This position is remote. Remote workers may be required to come on-site 0 to 12 days a year for work meetings, trainings, or for any other reasons that their supervisor may deem necessary (may include additional days for onboarding and new employee training). Supervisors will provide as much advance notice as practicable. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 4:30 p.m. Based on the business needs of this position, hires must live in or relocate to Minnesota.

This is a full-time, benefit earning position. You must be a current regular or probationary employee of Human Services Public Health (HSPH) to apply for this position. Limited duration employees are not eligible.

In this position, you will:

Lead and manage in a way that is approachable, transparent, and honest in order to establish trust and positive working relationships with the team and other supervisor peers as we work together to improve customer service and employee experience.
Collaborate and practice shared decision making and shared leadership when managing the initial assessment workflow and the transitions with critical partners in Initial Contact and Access, eligibility services, the service authorization unit, waiver finance team, office support, and transfer team.
Support team members by providing information, training, and coaching on program changes and new initiatives, and by providing feedback to individuals on what is working well and what needs to improve.
Provide leadership on county vision and mission, encourage team involvement in problem-solving individual situations for people we serve and systems issues, and promote frequent and transparent communication.
Develop, recommend, and implement policies and procedures ensuring compliance with human resources rules, state statute, state and federal programs, and assessment and case management best practices and standards.
Promote and maintain a work environment where all residents and employees are treated respectfully.

Need to have:

Bachelor's degree or higher from an approved school of nursing and two or more years of professional nursing experience.
Licensure as a Registered Nurse issued by the state of Minnesota.

Nice to have:

Certification as a Public Health Nurse in the state of Minnesota.
Experience working in a supervisory role.
Knowledge of:
- Assessment, planning, and service development for seniors and populations with brain injury, mental health, complex medical, developmental, and physical disabilities.
- County and community resources, practices of public health nursing, and public health ethics.
- Diversity and inclusion strategies with a focus on disparities reduction.
Ability to:
- Foster an environment that recognizes and respects cultural, lifestyle, and socioeconomic differences.
- Establish and carry out effective working relationships with employees, other professionals, agencies, and residents.
