Title of Position: RN / Specialized Care Nurse (Hospital to Home Program)

What You’ll Do and Who You Are:

Job Title: Specialized Care Nurse (RN)

Location: Peoria, Illinois

Position Summary:

The Specialized Care Nurse is responsible for delivering highly skilled nursing care to clients with complex medical needs in their homes. This role involves managing patients requiring specialized care, including those with ventilators, tracheostomies, and other advanced medical conditions. The RN will collaborate with multidisciplinary teams to develop and implement individualized care plans, ensuring optimal health outcomes and quality of life for clients.

Key Responsibilities:

Provide skilled nursing care to clients with specialized medical needs, including ventilator and tracheostomy care.
Develop, implement, and regularly update individualized care plans in collaboration with clients, families, and healthcare teams.
Administer medications and treatments as prescribed, monitoring for side effects and efficacy.
Educate clients and caregivers on disease processes, medication management, and self-care techniques to promote independence and understanding.
Maintain accurate and timely electronic health records, documenting assessments, interventions, and outcomes.
Communicate effectively with clients, families, and healthcare providers to ensure continuity of care and address any concerns or changes in condition.
