You use all resources available to them to manage the medical needs of our Home Health patients. You will work with other disciplines needed to maximize the patient’s outcome, be a patient advocate, and communicate with the primary Doctor, Clinical Team Manager, and other team members to manage the patient’s care. Qualifications:

Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
1 year nursing experience preferred.
Acceptance of philosophy and goals of this Agency.
Ability to exercise initiative and independent judgment.
