Major Responsibilities:

As an MDS Coordinator, you'll be an integral part of our team, overseeing and participating in the completion of the Resident Assessment Instrument (MDS), CAAs, and Care Plans in compliance with current Federal and State Regulations. Your key responsibilities will include:

Coordination of MDS assessments and the development of care plans in collaboration with the healthcare team, ensuring full compliance with regulations.
Management of Medicare and Managed Care residents, working closely with facility team members to provide comprehensive, personalized care.
Bringing innovative healthcare solutions to life by creating and implementing new ideas to improve processes, systems, and outcomes in patient care.

Your expertise and attention to detail will help us continue to deliver outstanding care to our residents.

Minimum Qualifications:

We are looking for a dedicated, compassionate professional with the following qualifications:

Active, unencumbered RN or LPN license in the state
Experience in Skilled Nursing/Rehabilitation facilities preferred
Six (6) months of experience as an MDS Coordinator
RAC-CT or RNAC certification preferred
Strong problem-solving and communication skills to work collaboratively with interdisciplinary teams
A commitment to compassionate, patient-centered care
