Job Description The Medical Coordinator is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $30.00 - $33.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential. The schedule for this position is Sunday - Thursday, 3:00pm to 11:30pm. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge. Link clients to medical services when necessary (insurance, PCP, specialists, referrals). Coordinate home health visits (if needed) and follow-up on medical appointments. Promote health literacy by providing patient education with regards to clients’ illnesses, medications, preventative measures, treatment management and overall general health. Arrange transportation for clients to and from medical appointments, as needed Report emergency situations to Supervisor/Manager and document incident Seek proper level of care to assess and treat medical complication Initiate medical response system when appropriate Support patient recuperation by providing necessary items and space needed to rehabilitate and assisting with the upkeep of the facility. Administrative/Record Keeping: Enter patient information and case notes into EHR and other documentation systems. Complete and update medication, vitals, progress notes and care plan Perform administrative tasks such as answering phones and filing paperwork. Prepare patient reports for billing. Maintain proper inventory of medical supplies and medications. Compliance/Safety: Maintain confidentiality in accordance with HIPAA guidelines, Illumination Foundation policies and DHS standards. Provide patients with proper compliance forms upon admission Utilize infection control measures according to OSHA standards. Conduct medication reconciliation and ensure proper disposal and pickup of bio-medical/pharmaceutical waste. Maintain a safe work environment by ensuring the cleanliness of the med room, wound care room, and medication storage room. Qualifications Required: Current and non-restricted LVN license in California 2 years of relevant experience. High School Diploma or equivalent. Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets. Valid CA Driver’s License. Preferred: Availability to work weekends and nights Home Health nursing experience and experience working with populations dealing with addiction and behavioral health conditions. Bilingual in English and Spanish. Proficiency in Google Suite (Mail, Docs, Sheets, Calendar) and KIPU or other electronic health record systems. Job Description The ECM Clinical Lead Care Manager is responsible for collaboration with the ECM team to serve homeless individuals with serious mental illness, substance use disorders and/or complex medical issues. The ECM RN/LVN will assist in addressing the social determinants of ECM smart goals. The ECM will collaborate closely with a multi-disciplinary care team to specifically include providing clinical support to Care Coordinators, Lead Care Managers and the entire ECM team of employees. They will measure progress towards the goals outlined in client care plans or assessments. The ECM will collaborate with a broader care team {PCP, BH clinicians etc.,) to ensure ECM enrollees are receiving the services outlined in the care plan. This is role requiring adherence to and application of evidence-based practices, knowledge of client and service barriers as well as social determinates of health, and providence of appropriate coordination of services to populations of focus. They help navigate health care services and systems, promote health and preventative care, and work closely with the client's Care Team. The pay for this role is $32-34 hourly. Responsibilities Provides clinical oversite to support Lead Care Managers/Care Coordinators. This includes chronic disease management and medication monitoring. Engages vulnerable populations as part of a multidisciplinary outreach team. This includes home visits, outreach to hospitals, homeless shelters and other settings, as needed. Provides clinical oversight and tracking of care pIans . Participates in the review of developed patient-centered care plans and reviews any updated care plans of all enrolled members. Works with hospitals to coordinate discharge plans with the behavioral health clinician, PCP, ACO, MCO etc. Performs timely medication reconciliation following transitions in care. Supports medication adherence. Use data to evaluate outcomes from targeted interventions of clients. Assists in developing appropriate adjustments to care plans based on this data. Implements prevention & engagement activities. By presenting case conferences at scheduled monthly/weekly meetings. Provides health coaching for groups of enrollees or individual enrollers as needed. Engages in quality improvement efforts. By participating in audits by various program administrators. ECM RN/LVN shall attend appropriate disciplined training, webinars, meetings and conferences. Promote monthly Health Promotion topics/materials. Complete care plans and utilize Electronic Health Record{s) {EHR) and client databases {i.e., MIS, Champ, or Health plan programs if applicable) to track and maintain accurate case notes, client, and program data in SMART Format where applicable. Engage with the Client care Team by participating in weekly Interdisciplinary Care Team meetings to evaluate program effectiveness and client progress, while developing new strategies to address clients' needs and enhance care plans Conduct weekly and/or bi-weekly care team meetings with all relevant members of the client's care team, in addition to monthly meetings, to review progress and coordinate care. Preferred: • Bilingual in English and Spanish. • At least 2 years of experience working with at risk/unhoused individuals • Proficiency in Microsoft Office (Outlook, Word, Excel, Teams).
