12 hour shifts Night Charge Nurse Night Shift differential Pay Job Summary: We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at Cedar Ridge Inn. In this role, you will provide direct patient care to residents, supporting their daily healthcare needs and collaborating with the nursing team to ensure optimal care. The ideal candidate will have a strong clinical background, excellent communication skills, and a genuine passion for caring for others in a long-term care or healthcare setting. Key Responsibilities: Resident Care & Monitoring: Provide routine nursing care for residents, including monitoring vital signs, assisting with activities of daily living (ADLs), and observing and reporting changes in residents' conditions. Medication Administration: Administer prescribed medications accurately and monitor for potential side effects or adverse reactions. Document medication administration and any related observations in residents’ medical records. Wound Care & Treatments: Assist with the care of wounds, dressings, and other medical treatments as ordered by the healthcare team. Ensure proper infection control protocols are followed. Care Plan Support: Assist in the development and implementation of individualized care plans for residents. Monitor the effectiveness of care plans and report any changes or concerns to the RN or physician. Documentation & Reporting: Document all nursing care provided, including assessments, medications, treatments, and observations. Ensure that records are updated and comply with facility policies and regulatory standards. Patient and Family Education: Provide education to residents and families regarding health conditions, treatments, and medication management. Offer guidance on improving overall health and well-being. Collaboration & Communication: Work closely with RNs, physicians, CNAs, and other healthcare professionals to ensure coordinated care for residents. Communicate any concerns or changes in residents’ health status to the appropriate team members. Emergency Response: Respond quickly and effectively to any emergencies, providing necessary interventions and communicating with physicians and other team members. Supervision: Oversee and provide guidance to Certified Nursing Assistants (CNAs) and other support staff, ensuring that care plans and procedures are followed appropriately. Qualifications: Licensed Practical Nurse (LPN) with active licensure in New Mexico. Minimum of 2 years of nursing experience, preferably in a long-term care, skilled nursing, or healthcare setting. Strong clinical skills and understanding of nursing care principles, procedures, and regulations. Excellent communication and interpersonal skills with the ability to work effectively with residents, families, and the healthcare team. Ability to prioritize tasks, manage multiple responsibilities, and provide quality care in a fast-paced environment. Registered Nurse (RN)

Key Responsibilities: Resident Assessment & Care: Conduct comprehensive assessments of residents upon admission, throughout their stay, and when changes in condition occur. Monitor and evaluate residents' health status, identifying potential issues and implementing appropriate interventions. Care Plan Development: Develop, implement, and update individualized care plans for residents. Work with the healthcare team to ensure that care plans reflect the residents’ needs, preferences, and medical conditions.
