About the Role:

As an integral member of our interdisciplinary care team, you will play a vital role in assessing resident needs and developing individualized care plans. Your responsibilities will include:

Conducting comprehensive MDS assessments accurately and efficiently.
Analyzing resident data to identify care needs and develop appropriate care plans.
Participating in care planning meetings and communicating effectively with residents, families, and other healthcare professionals.
Ensuring compliance with all regulatory guidelines and best practices.
Maintaining accurate and up-to-date resident records.
Promoting a resident-centered approach to care.

Qualifications:

- Current RN or LPN licensure in the state of Indiana.
- Minimum of 1 year of experience in long-term care or skilled nursing.
- Strong assessment and documentation skills.

- Excellent communication and interpersonal skills.

Ability to work independently and as part of a team.
Passion for providing high-quality care to seniors.
