KeystoneCare is seeking a compassionate and dedicated Hospice Face-to-Face Nurse Practitioner to join our team. In this pivotal role, the Nurse Practitioner will conduct face-to-face assessments for patients receiving hospice services in alignment with Medicare requirements. You will play a critical role in ensuring that patients receive the appropriate level of care and support during their end-of-life journey.

Your expertise will help bridge the gap between patients, families, and the healthcare team, providing a holistic approach to care that addresses physical, emotional, and spiritual needs. You will collaborate closely with other healthcare professionals and families to develop and implement patient-centered care plans that honor the values and preferences of each patient.

Primary Job Responsibilities:

Conduct face-to-face assessments for hospice patients to determine eligibility for hospice care
Evaluate patients' conditions and update care plans as necessary to ensure compliance with all regulatory requirements
Collaborate with the hospice interdisciplinary team, including physicians, nurses, social workers, and spiritual care providers
Communicate effectively with patients and their families to provide guidance and support in the hospice care process
Document assessments and findings accurately in a timely manner to ensure continuity of care
Educate patients and families about hospice services and provide resources to support their needs

Graduate of an accredited nurse practitioner program
Current Nurse Practitioner license in good standing to practice in the Commonwealth of Pennsylvania
Minimum of two years of clinical experience in hospice or palliative care settings preferred
ACHPN certification preferred or the ability to obtain within six months of hire
In-depth knowledge of hospice regulations and Medicare policy
Ability to communicate compassionately and effectively with patients and families during challenging times
Strong organizational skills and attention to detail
Valid driver's license and reliable transportation required
Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
