Job Overview:

Falcon Point Post Acute, a leading provider of long-term care and skilled nursing, is seeking an experienced Unit Manager & Infection Control Nurse to join our dedicated healthcare team. This dual-role position oversees the daily clinical operations of a designated unit while ensuring infection control protocols are implemented and followed across the facility. The successful candidate will have a passion for quality care, leadership, and infection prevention, ensuring a safe and healthy environment for both residents and staff in our general and secured units.

Key Responsibilities:

Unit Manager Responsibilities:

Oversee Unit Operations: Manage the daily functions of the assigned unit, ensuring the delivery of high-quality nursing care to residents in compliance with regulatory guidelines.
Supervision of Staff: Provide leadership, supervision, and support to nursing staff, including RNs, LPNs, and CNAs, ensuring effective communication and teamwork.
Care Planning: Collaborate with the interdisciplinary team to develop and implement individualized care plans for residents, ensuring all health and safety needs are met.
Resident & Family Interaction: Serve as the main point of contact for residents and their families, addressing concerns, facilitating care meetings, and promoting resident satisfaction.
Documentation: Ensure accurate and timely documentation of resident care in compliance with state, federal, and facility policies.
Regulatory Compliance: Stay current on all state and federal regulations, ensuring the unit is survey-ready and compliant at all times.

Infection Control Nurse Responsibilities:

Infection Control Program: Develop, implement, and maintain an effective infection prevention and control program for the facility, in accordance with CDC, CMS, and state guidelines.
Surveillance & Monitoring: Conduct regular infection control rounds, monitor infection trends, and initiate interventions to reduce infection risks.
Education & Training: Provide ongoing education and training to staff on infection prevention protocols, hygiene practices, and the use of personal protective equipment (PPE).
Policy Development: Develop and update infection control policies and procedures to ensure they reflect current best practices and regulations.
Outbreak Management: Coordinate responses to infection outbreaks, including isolation protocols, resident monitoring, and communication with local health authorities.
Audits & Reporting: Conduct regular audits of infection control practices and prepare reports for the leadership team and regulatory agencies as required.

Qualifications:

Licensure: LVN/RN with an active license
Experience: Minimum of 1 years of experience in a skilled nursing or long-term care setting, with experience in unit management and/or infection control preferred.
Infection Control Certification (CIC): Preferred, or willingness to obtain within a specified timeframe.
Leadership Skills: Proven leadership ability with experience supervising nursing staff in a healthcare setting.
Knowledge of Regulations: Strong knowledge of federal, state, and local regulations regarding skilled nursing, long-term care, and infection control.
Excellent Communication: Ability to effectively communicate with residents, families, and staff at all levels.
Organizational Skills: Strong organizational and time management skills, with the ability to handle multiple priorities in a fast-paced environment.
Problem-Solving Ability: Ability to think critically and make informed decisions under pressure.
