Admissions Nurse (RN) Opportunity at Crawford Care Center

Position Details:

Schedule: Interim 60 days

Active Pennsylvania RN license

Key Responsibilities:

Serve as the initial point of contact for patients during the admissions process.
Conduct pre-admission screenings, including a comprehensive medical assessment and history collection.
Ensure completion of all required forms and insurance-related documentation.
Facilitate a smooth transition for patients into the facility, addressing any concerns or anxiety.
Perform patient assessments upon admission and develop individualized care plans in collaboration with the healthcare team.
Communicate effectively with patients and families, offering support and guidance.
Maintain accurate and timely documentation of patient records.
Coordinate and streamline the admissions process.
