Home Health Aide (DEPT only)
The Home Health Aide –is a non-licensed, member of the home health care team. The HHA –assists the client with personal care activities as outlined in a written plan of care that is established by a Registered Nurse or Therapist and kept in the client’s EMR. Home Health Aides are supervised by a RN and receive scheduling direction from an agency scheduling coordinator.
High school diploma or general education degree (GED) required
Must be at least 18 years of age.
Current at upon hire - Michigan State Certification of a Nurse Aide/Home Health Aide.
Must possess current Michigan driver’s license and carry proof of auto insurance.
- Good organizational skills.
- Has ability to read and follow written instructions and document care provided.
- Is self-directing with the ability to work with little direct supervision.
- Is secure with issues of death/dying.
- Has empathy for the needs of the ill, injured, frail and the impaired.
- Demonstrates tact, patience and good personal hygiene.
- Able to project a calm manner.
- Ability to read and write, speak and comprehend English.
- Second language is desirable but not necessary.
- Must be able to successfully and independently access any type of a private residence.
- Regularly lifts or manipulates weights in excess of 100 pounds.
- Has near visual acuity and functional hearing is required for accurate assessment and recording of patient observations, and information on clinical records.
- Good oral and written communication skills
- Under direction of the patient’s physician and RN case manager, gives emotional support, comfort, and encouragement necessary to assist patient/family in achieving agreed upon goals of care.
- Effectively utilizes established communication structures and processes (meetings, memos, written policies and procedures, telecommunication tools, etc.) to facilitate optimal performance and service delivery.
- Maintains and respects confidentiality of client and agency information.
- Communicates well with care team and management.
Functions effectively as a team member.
Demonstrates understanding of own role and roles of others. Actively supports co-workers.
Demonstrates commitment to mission and goals of Bronson at Home.
Is able to focus on what needs to be accomplished to be successful as a caregiver and as a team.
- Collaborates with others to facilitate continuity of care by all providers within assigned caseload. Volunteers to assist co-workers as needed.
- Attends/participates in case conference meetings when appropriate
- Participates in agency in-services and clinical record reviews.
Willingly assists with assignments when co-worker calls off work unexpectedly, or to travel further distances to assist with patient care needs.
- Treats internal and external customers with respect on the phone or one on one.
- Responds to customer concerns in a professional and friendly manner.
- Utilizes customer complaint and incident report forms and processes to assure investigation and resolution of customer concerns.
- Appearance and clothing is clean, neat, and appropriate for job responsibilities.
- Accurately adheres to the Home Health Aide Care Plan.
- Performs personal care according to client’s Care Plan including recording pertinent information pertaining to the client’s care.
- Accurately prepares daily records
- May measure and empty drainage bags and measure weight as ordered.
- Assists client in performing personal care including hair care, oral hygiene, bathing, grooming and dressing.
- Takes and records, oral, rectal and auxiliary temperature, pulse, respiration and blood pressure when ordered (within the training limitations of the aide).
- Provides proper care and observation of client’s skin to prevent breakdown of tissue over bony prominence.
- Assists client with safe ambulation with or without aids.
- Assists client with therapy exercises under the direction of a Registered Therapist. Meets safety needs of client using equipment safely and properly (foot stools, side rails, water, etc.)
- Observes and reports changes in client’s condition to Case Manager.
- Completes all required documentation accurately and within agency deadlines.
- Participates in in-service education programs.
- If unable to work calls supervisor/delegate promptly.
