Job Summary
The Anticoagulation Clinic nurse practitioner lead is responsible for the care of patients in the Anticoagulation Clinic. In collaboration with clinical pharmacists, the nurse practitioner supervises anticoagulation therapy for ambulatory patients through ongoing clinical assessment, ordering and following up on laboratory testing, and modifying medications as needed to meet anticoagulation goals.
They collaborate with other disciplines or departments to provide care coordination involving anticoagulation management, including perioperative departments, home health, and other procedural areas.
The nurse practitioner provides patient education, verbally and in writing, regarding anticoagulation medications and indications for use, medication side effects or precautions, and laboratory monitoring timing and follow up.
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $95.69 - $124.10 (Hourly Rate).

Required Qualifications
- Minimum two (2) years managing patients on long-term anticoagulation therapies as a nurse practitioner within the last three (3) years
- Minimum one (1) year of active ambulatory clinical experience as a nurse practitioner within the past two (2) years
Preferred Qualifications
- Experience with EPIC clinical documentation
License/Certification
- California driver’s license (CDL) if required for position.
- Current national certification from an accepted certifying agency and provide by first day of work.
- Accepted Certifications for this position: AGNP-C or A-GNP-C, ANP-C, FNP-C, GNP-C, AGACNP-BC, AGPCNP-BC, ACNP-BC, ANP-BC, FNP-BC, GNP-BC, ACNPC-AG or ACNPC. Link to UCSF NP Certification Table for reference: Acceptable NP Board Certifications Checklist 2.23.24.docx
- Complete a BRN recognized graduate master’s degree program as a nurse practitioner.
- Obtain an active and/or transferable DEA license to California and provide evidence by 90th day of clinical practice. (It is the responsibility of the employee to transfer their DEA license to their new place of employment if from out of state, via online DEA portal and provide evidence of transfer).
- Submit enrollment with CMS Medicare and Medicaid through credentialing by first clinical day. CCS enrollment as appropriate for pediatric environments.
- Current AHA BLS as minimum requirement.
- State regulation requires that you have a supervising physician to practice.
- Incumbents with less than 6 months of NP experience will be hired as a Nurse Practitioner 1.
UCSF Medical Center and Campus requires certain healthcare providers to be credentialed. In most cases this is done through its Office of Medical Affairs & Governance (OMAG) prior to the first working day. Credentialing and approval of the Standardized Procedures takes between 60-90 days but can vary.
