Must be a RN with MDS experience

Position Summary:

The MDS Nurse Coordinator is responsible for the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines.

Job Responsibilities

Identify resident acuity and needs, help to determine specific care needs and communicate needs / expectations to families and responsible parties
Complete assessments, PDPM and care plans for all residents assigned
Monitor completion of PDPM's by other disciplines within time frames prescribed by regulatory guidelines
Advise supervisor of incomplete and/or untimely assessments by disciplines other than nursing
Ensure accurate, timely completion of the Triggers sheet for assigned residents
Initiate care plans and supporting activities that will result in best possible outcome for assigned residents
Monitor and ensure completion of 7-day look-back documentation by nursing staff within prescribed time frame
Maintain current knowledge of PDPM assessment process
Run the Restorative program in the community
Responsible for Quality Measure Program

Qualification

·Licensed RN or LPN with the state of Illinois

·2 years MDS and Care plan experience

·Knowledge of Medicare MDS and PDPM

·Experience in Restorative Programming, Fall care, monitoring and investigation prevention

·Knowledge of Medicare and Medicaid reimbursement
