Job Summary:
Provides expert patient care and acts as a clinical resource to staff. Assists nursing staff in the development, implementation and evaluation of nursing care and treatment for patients in an assigned area. Enhances the patient experience by providing a seamless navigation process from diagnosis through end of care. Responsible for collaborating with physicians and members of the multidisciplinary teams to triage, coordinate, and consistently manage patient care by assuming the role of resource person for the patient and the multidisciplinary team. Works with patients and families to answer questions, provide emotional support and solve issues concerning the cylce of care.
Education & Experience
Must be licensed as RN for two (2) years.Specialty or case management experience preferred.
CERTIFICATIONS, LICENSES OR REGISTRATION REQUIRED: Valid RN license.
Knowledge, Skills & Abilities
Knowledge of clinical nursing care procedures and protocols. Verbal and written communication skills. Interpersonal skills. Knowledge and understanding of computer systems, tools and programs. Ability to work in a team environment.
Responsibilities
- Navigates patient through the continuum of care. Possesses significant knowledge of pediatric are to ensure patients are directed to care required post ED discharge.
- Educates patients (and possibly family members) on topics such as the nature and status of patient's condition and care deliver goals based on patient specific care plan. Contacts families based on post discharge results/needs and facilitates changes in care plans.
- Serves as liaison for patients/families, providers, nursing and other appropriate staff. Monitors post discharge lab results and radiological reads and outpatient studies.
- Consults and reports and patient issues and concerns to other medical and health care staff.
- Provides patient care by assessing, planning, implementing, and evaluating treatments and procedures for patients. Ensures consults and treatments are correct based on plan of care.
- Provides a visible and specific channel through which patients can seek solutions to problems concerns, and unmet needs. Handles post discharge questions from families related to care (prescription issues, work/school excuses, lab follow ups, etc.).
- Maintains accurate patient records (e.g. charts, schedules, procedures, medication/treatment history, etc.) Tracks missed reads, reports errors and reports to physician leadership.
- Maintains clinical competency and provides direct patient care as needed. Staffs in all roles needed within the department- charge nurse, triage nurse, bedside nurse, etc.
