Recognized as a patient navigation expert. Serves as an expert clinical resource and liaison for patients and their families. Navigates all aspects of the specialized area within the healthcare system. Provides specialized clinical education in regards to treatment and liaisons with the patient's clinical service providers to address specific concerns. Has in-depth specialty expertise and focus.
Under the direction of the Manager of Care Transitions Programs and the Medical Director for Transitions and along with other Population Health Clinical Program Leadership, this Lead Nurse Coordinator will be an integral member of the Office of Population Health Care Transitions and Relay Center Team. The Lead Nurse Coordinator serves as the UCSF-wide expert on care transitions from the Emergency Department and results reporting.Recognized as a patient navigation expert, the Lead Nurse Coordinator is responsible for maintaining and updating result follow-up and care coordination workflows across UCSF Health Emergency Departments, inpatient and ambulatory clinics. He or she serves as an expert clinical resource and liaison for patients and their families.
The Lead Nurse Coordinator establishes and maintains relationships with key stakeholders, facilitating meetings with multidisciplinary providers to present optimize effectiveness and efficiency of the Relay Center. He or she serves as the team leader and key partner with UCSF Emergency Departments. The Lead Nurse Coordinator designs and implements supportive interventions for clinically or financially high risk patient populations in need of specialized care. He or she serves as the team liaison with outside stakeholders, including the CipherHealth team.
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $113,500 - $258,100 (Annual Rate).

Department Description
The Office of Population Health (OPH) leads UCSF’s development of innovative care models to improve the value and quality of care and improve health outcomes for identified populations and facilitates implementation of these care models across the health system. OPH is responsible for fulfilling UCSF Health’s accountable care contracts and fostering care partnerships across a clinical network. This department is comprised of four main teams: (1) Clinical Programs, (2) Population Health Quality and Performance Improvement, 3) Ambulatory Risk Capture/Medicare Advantage Operations, and 4) Population Health Administration.
Required Qualifications
- At least three years of experience working as a Patient Navigator Clin 4, or as a nurse in an Emergency Department
- Graduate form an accredited school of nursing, BSN is required.
- Current CA license to practice as a Registered Nurse
- Active AHA Basic Life Support (BLS) certification
- Expert clinical knowledge in a variety of modalities, or significant expertise in a specialty area. Expert ability to assess patient needs and make appropriate referrals for specialized support services and programs.
- Highly developed skills in assessing patient's grievances and locating appropriate resources to get issues resolved. Highly developed skills to intervene and assist with patient's questions or concerns about treatment plan.
- Highly developed patient care skills, working with a high volume and high expectations customer base. Strong ability to support patients with a wide variety of clinical and non-clinical needs.
- Highly-developed skills and expertise working in a complex, multi-divisional environment, with proven ability to initiate projects and lead teams, prioritize tasks, and direct projects successfully from inception to completion on task and on schedule.
- Highly-developed communication skills to teach patients about complex, sensitive clinical issues.
Preferred Qualifications
- Demonstrated knowledge with Windows-based software including Microsoft word, Excel, Outlook. Strong Knowledge of clinical based computer systems and software.
- Certification in Geriatrics, Palliative Care or Case Management in Nursing
License/Certification
- Current CA license to practice as a Registered Nurse
- Active AHA Basic Life Support (BLS) certification
