Registered Nurse

A Place at Home, Somerville, NJ

Key Responsibilities

Assess and evaluate the needs of patients and develop an appropriate plan of care.
Provides education and strategies for patients and families.
Provides education, strategies and guidance to agency's Home Health Aides.
Documents all changes in patient’s condition and updates care plans and outcomes.
Some office work and other projects as needed.

This job description is not exhaustive. You may be expected to perform other reasonably related duties as assigned by management. Ideal position for retired RN looking to make a difference in the Senior Community.

Skills/Qualifications

- RN Current License.

- Experience in home care or related field prefered but not required.

- Ability to educate patients and families.

- Ability to collaborate with other healthcare professionals.

- Knowledge of safe work practices in a home care setting.

- Proficient documentation skills.
