Summary
Under general direction of nursing leadership, the supervisor RN position is responsible to assist home care nursing manager with planning, organizing, and directing of the operations and staff of home care nursing and home health aides. Direct supervisor for all home care health aides and nurses under assigned branch. Leads assigned home care nursing, home health aides, and activities to accomplish the goals of the organization by means of motivating, educating, guiding and communicating. This position works under home health nurse manager to assist with planning, directing, coordinating, and evaluating nursing care to ensure optimal patient health outcomes in home care. This position links nursing leadership with nursing care through the application of organizational and nursing service policies, procedures, and care delivery model. The Supervisor RN uses the Code of Ethics for Nurses with Interpretive Statements to guide leadership and practice. The Supervisor RN attains knowledge and competence reflective of current nursing practice; promotes innovation and futuristic thinking by incorporating evidence-based practice and research findings.
Responsibilities
(30%)* Directs all patient care activity within the assigned home care branch including on-going evaluation, treatment, and discharge planning. Establishes patient care delivery goals and objectives that are consistent with the organization's mission. Provides direct patient care when needed and appropriate.
(25%)* Responsible for hiring, disciplining and discharging employees within home care direct report assignments according to medical center policy. Responsible for written Job Performance Reviews (JPR's) for all direct reports.
(25%)* Works with home health nursing manager to determines and maintains the staffing and schedule patterns to provide 24 hour/day, 365 days/year coverage of home care.
(20%)* Collaborates with physicians, all levels of nursing management, and other home care and departmental personnel in planning for coordinated services to the patient. This includes therapy, social work, acute care telehealth nurses, intake, billing, quality review medical records, and information technology.
OTHER DUTIES AND RESPONSIBILITIES:
Maintain expertise in OASIS to teach and support home care clinicians.
Maintain expertise in home health EMR to teach and support clinicians and to monitor clinical documentation supports required criteria and billing requirements.
Maintain expertise in how home care intake documentation, clinical documentation, quality review processes, and patient accounting all interface with downstream effects. Ongoing monitoring of assigned work ques to hold clinicians accountable and to address clinical issues impacting billing.
Continuously monitor quality measures such as patient satisfaction and star rating and facilitate quality improvement measures and indicated.
Assures compliance with all policies, procedures, regulations, standards and laws such as but not limited to Joint Commission, Centers for Medicare and Medicaid Services (CMS) and Michigan Department of Public Health (MDPH).
Designs and conducts Process Improvement and Standards of Care for home health and hospital at home unit including establishing and writing policies and procedures for home care.
Works with direct reports in nursing and home health aides along with patient support networks and caregivers to ensure customer satisfaction, address complaints/concerns in order to meet their individual needs.
Takes an active role in regular nursing management meetings to influence the direction of nursing service.
Actively leads/participates in interdisciplinary conferences and huddles.
Participates on or leads nursing service committees as appointed by Director of Home Care.
Plans, develops, implements and evaluates educational programs for orientation, staff development, and continued growth of all employees within home care.
Counsels staff as needed.
Responsible for maintaining lines of communication with peers, subordinates and superiors.
Proactively works with patients and caregivers to ensure customer satisfaction, address complaints/concerns in order to meet their individual needs.
Monitor ordered nursing frequency and ensure plan of care appropriate, approve recertifications, ensure timely discharges to maximize capacity.
Participates in home care leadership on call rotation to support two home health organizations, home infusion, home hospice, the Woodland Hospice House and hospital at home units in two medical centers.
Ensures all assigned branch locations maintain adequate medical supplies, admission books and forms, etc. Approve home health supply orders outside of formulary.
Assist with home care community fundraisers such as Butterflies in the Park, Stars in the Garden, etc.
Travels between multiple counties and to assigned branches for routine rounding on direct reports and to compete operational and environmental rounding in assigned branch.
Develop strong partnerships with skilled nursing facilities, assisted living facilities, medical centers, etc.
Assist with home care marketing and community presentations as indicated.
Must be able to drive in less than ideal weather conditions.

EXPERIENCE, TRAINING AND SKILLS:
Knowledge of nursing practices and procedures as normally acquired through 2 years experience working in a home care or similar setting. Will consider acute care. Preferred knowledge of nursing leadership as normally acquired through working 2 years in supervision, project implementation, professional development program, shared governance leadership role, or equivalent.
Leadership skills needed to effectively direct a group.
Interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understandable manner.

Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.
Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.
