Job Requirements

Summary:

Facilitates and supports the development and implementation of clinical initiatives within the communities; Provides support, guidance, and direction to the clinical teams in the assessment, planning, implementing and the evaluation of resident care; Collaborates with the “Directors of Nursing” to establish and implement standards of practice in the delivery and documentation of care; Provides expertise and oversight for the RAI process and Medicare program compliance.

Essential Duties and Responsibilities:

1) Serves as an “internal consultant” to LSS communities’ clinical and clinical services departments or individuals. Helps to analyze and determine possible solutions to issues in the department’s clinical operations and makes recommendations to the Executive Director, Administrator and/or Director of Nursing

2) Provides direction and oversight of clinical systems and support communities in establishing systems and plans of action

3) Translates clinical programs to workflows for applicable staff

4) Collaborates with the Director of Clinical Compliance and Center of Clinical Excellences team on the clinical standards of practice for LSS communities, in association with “Directors of Nursing” (DON) and “Directors of Health Services” (DHS), and assists communities in having their practice consistent with state and federal regulations governing licensed communities in Missouri and Illinois

5) Creates a model for consistent implementation of the clinical practices for LSS communities to ensure compliance with the clinical process, documentation procedures, care delivery systems and products used in the care of residents at all LSS communities

6) Supports care delivery to ensure positive clinical outcomes, utilizing CMS QI/QM Reports, EHR, Power Bi, MDS, and clinical audits to track and trend

7) Collaborates with DONs and DHSs to ensure that clinical policies and procedures for the clinical departments are consistent with Federal and State guidelines and are consistent with accepted clinical practices in the community

8) Provides support with preparation for Annual Survey, during Annual and Complaint Surveys and with plans of correction

9) Assists in the orientation of new DON’s and DHS’s and provides ongoing support

10) Must maintain a thorough and up-to-date understanding of all regulations and standards related to skilled nursing, including the RAI process

Knowledge, Skills, and Abilities:

· Current RN nursing license for the state employed in

· Must be CPR BLS (basic life support) certified with the American Red Cross

· Graduate of an accredited school of nursing, BSN preferred and MSN preferably

· Minimum of 5 years relevant clinical experience

· Minimum of one-year acute care and greater than 5 years post-acute care nursing experience

· Knowledgeable of the rules and regulations related to long term care/skilled clinical including the RAI process and Medicare A
