Summary

Responsible for processing new referrals; determining appropriate coverage; communicating with patients and referral sources; and reviewing clinical documentation at Start of Care.

Responsibilities

Answers incoming referral calls in a timely manner and with positive communication skills

Elicits accurate, thorough information from referral sources

Responsible for determining valid Face to Face encounter on all referrals

Inputs initial information into computer to begin the admission process

Notifies appropriate clinicians for timely start of care/resumption of care

Enters physician orders for initial visits

Contacts patients by phone to inform them of start of care date

Pre-screens patients over phone for eligibility for Home Health services

Documents information on referrals not opened

Obtains and enters insurance authorizations as necessary

Reviews all case openings for completion and compliance

Reviews ICD-9 codes

Tracks and provides follow up on hospitalized patients

Reports weekly census to Marketing Office

Reviews on-call log and follows up as necessary

Attends community meetings, staff meetings and in-services as required

Performs duties of staff nurse as needed

Serves as the administrative person – on-call as scheduled or as the on-call RN as scheduled

Qualifications, Knowledge, Skills & Abilities

Minimum of one year of home health experience.
Graduate of an accredited school of nursing.
Must be CPR BLS (basic life support) certified with the American Red Cross
Minimum of 2 years nursing experience.
Ability to provide own transportation.
Current Missouri and Illinois State Nursing License.
Proficient in computer skills.

Noise level in work environment is usually quiet.
Travels out in the community that allows for exposure to weather conditions and potential for unsafe environments.
