A Brief Overview
The Health Connections Staff Nurse assumes responsibility and accountability for a group of patients for a designated time frame and provides care to these patients. Using the nursing process, the consultation nurse facilitates access to the health care system and/or negotiates a satisfactory resolution to the client's problem. The Health Connections Staff Nurse is accountable for quality assessment, efficient managed care, and client satisfaction in collaboration with the health care team. The HC nurse gathers patient information, health history, verification of orders, and scheduling if needed for outpatient procedures including surgical procedures and documents all information within EPIC. She/he is also responsible to make outbound calls for patients with outpatient procedures to assess outcomes, reinforce discharge instructions, and patient satisfaction. The Health Connections Staff Nurse is expected to work closely with providers, nurses, and all other members of the organization by demonstrating effective communication, teaching, and negotiating skills. The delivery of professional nursing care at Stormont Vail Health is guided by Jean Watson's Theory of Human Caring and the theory of Shared governance, both of which are congruent with the mission, vision, and values of the organization.
Education Qualifications
- Bachelor's Degree Bachelors of Science: Nursing (BSN) Preferred
Experience Qualifications
- 2 years Ambulatory nursing, or medical surgical nursing experience. Required
- Experience in a broad base of nursing. Preferred
- Experience in telehealth or providing healthcare over the phone. Preferred

Licenses and Certifications
- Registered Nurse - KSBN Required
- Additional certification may be required for certain patient care settings or populations. The department director will provide specific information and the timeframe in which such certification must be attained at the time of hire/transfer into department.
What you will do
- Through the nursing process, using specific protocols and established resources: collect specific data and records information in database/documentation form, assesses the client’s condition, provides accurate consistent information, formulates and recommends a plan of care, negotiates appropriate nursing care options with the client, and validates the care plan with the client for completeness and accuracy.
- Provides patient care as defined within the scope of the state’s nurse practice act. Performs assessment/data collection in an ongoing and systematic manner, focusing on physiological and cognitive status.
- Assists patients and callers over the phone, answers questions and records information in the system. Performs follow-up of telephone contacts with appropriate daily correspondence to client/patients and physicians.
- Triages callers properly such that all callers receive a timely and appropriate response. Continually works to increase the number of calls taken/made without sacrificing quality.
- Is responsible for managing callers electronic information. Updates and maintains current data in the electronic medical record. Maintains adherence to data policies regarding patient confidentiality.
- Reports pertinent observations and reactions regarding client to the appropriate person (i.e. supervisor, department director, and/or provider).
- Evaluates the patient’s progress towards achieving outcomes.
- Performs efficiently in emergency patient situations following established standards of care. Practices effective problem identification and resolution as a method of sound decision making.
- Participates in system wide/department patient care quality improvement activities as well as standards development.
- Participates actively in clinical education activities, including orientation of new employees and teaching student nurses. Participates in nursing research activities.
- Participates in development and attainment of system wide/department hospital goals.
- Delegates tasks and duties in the direction and coordination of health care team members, patient care and department activities in accordance with the Kansas State Nurse Practice Act.
- Adheres to staffing and scheduling practices.
- Seeks validation of knowledge base, skill level and decision making as necessary and assertively seeks guidance in areas of question.
