Summary

Come work at Polaris, a newly renovated 100-bed skilled nursing facility with a state-of-the-art vent unit located at the heart of Milford Wellness Village in Milford, DE. The Milford Wellness Village is a campus that provides a wide range of services that address the healthcare needs of the Milford community.

Milford Wellness Village is unique in that it offers a safe and well lit parking area with onsite security; not to mention onsite day care and café for all to enjoy.

We are seeking highly skilled, enthusiastic, & passionate Licensed Practical Nurses (LPN) to join our compassionate community and provide exceptional patient care.

Key Responsibilities for LPNs:

LPNS take an active role in direct resident care and development of care plan.
Administers medication and/or treatment to residents
Communicates resident pertinent data to the appropriate responsible party.
Supervises and coordinates certified nursing aides in providing direct resident care in adherence to State, and Federal guidelines.
Ensures that residents receive the highest quality of service in a caring and compassionate atmosphere.
The ability to be a flexible, caring, supportive team member

Qualifications:

LPN license in the state of Delaware
CPR Certification
Experience is preferred in long term care
Specialties in rehab, geriatrics, and or ventilator care a plus
Ability to build rapport with residents
Compassionate and caring demeanor
