Jacksonville, FL 32257 Under the supervision of the Triage Supervisor and the direction of the Triage Nurse, will make visits after hours to patients in various settings for assessment, symptom management, treatment, teaching and coordination of care. Makes visits at time of death when appropriate. Work hours are 830p-830a on a rotating schedule covering 7 shifts in a 14-day period.

Primary Responsibilities:

Completes assessment of patient during visits for effective symptom management.
Perform treatments/interventions in accordance with the Plan of Care and physician’s orders.
Provides emotional support to patient/caregiver.
Teaches patient/caregiver as appropriate to plan of care.
Follows Standard Precautions.
Reports to Triage Nurse following patient visits.
Leaves accurate and detailed messages in voice mail boxes for all appropriate team members after making all visits including death visits.
Accurately completes documentation of visits/phone communications and submits documentation in a timely manner per policy.
Calls physician as necessary for change in Plan of Care.
Facilitates change to continuous care when necessary as per policy.
Follows policy when patient is transferred to acute care after hours.
Arranges for medications and supplies in an emergency per policy.
Answers pager and/or phone calls within 15 minutes.
Develops basic knowledge of Hospice Medicare/Medicaid and other reimbursement mechanisms.
Develop areas of expertise in hospice nursing skills.
Pursues professional growth through academic and continuing educational programs.
Accepts responsibility and accountability for own actions.
Demonstrates ability to solve problems independently.
Seeks supervision appropriately.
Organizes workload efficiently.
Accepts criticism non-defensively.
Handles changes appropriately.
Functions effectively in emergency situations.

Displays courtesy, tact and patience.
Maintains a good attendance record.
Notifies agency of absences according to policy.
Keeps neat and legible written documentation.
Participates in agency quality assurance and complies with Compliance Program.
Attends staff meetings on regular basis.
Other duties as assigned

Required Licenses/Certifications:

Must currently be registered in the State of Florida as a Registered Nurse.
Current State of Florida drivers license with driving record acceptable to Community Hospice insurance carrier, and reliable transportation

Qualifications:

Education/Experience:

Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying.

Typical qualifications would be equivalent to:

Possession of a degree from a diploma nursing program, 2 year Associate Degree, or Bachelor’s Degree in Nursing.
Minimum of two (2) years experience in acute care setting, oncology, home health setting, and hospice preferred.
Knowledge of principles and practices of nursing.
