Job Summary: In accordance with departmental and organizational policies, procedures and guidelines, the Clinical RN is responsible for the clinical coordination of patients enrolled in the Palliative Care program, assisting in aligning care with the patient’s goals. The Clinical RN will have expertise in pain and symptom management to best facilitate discussions with the patient, their significant others, and the healthcare team. Complies with regulatory and professional standards, fosters interdisciplinary collaborative relationships within the department, and works closely with physicians and healthcare team members in establishing a collegial and interdisciplinary patient care environment. Licensure and Certification: Current California RN license Current BLS provider card Advanced Certified Hospice and Palliative Nurse (ACHPN); or Certified Hospice and Palliative Nurse (CHPN) preferred Education: Associate’s degree(ASN) in Nursing is required Bachelor’s degree (BSN) in Nursing is strongly preferred Member in a professional organization preferred End of Life Nursing Education Consortium (ELNEC) preferred Experience: Three (3) years current nursing experience. At least three to five (3-5) years of professional nursing experience in patients with chronic life-limiting illness, with demonstration of competent clinical judgment and leadership ability. Ability to work effectively with physicians and a multidisciplinary team toward achieving a common goal. Knowledge and Skills: Experienced, clinically competent RN able to apply an in-depth understanding of patho-physiology and advanced nursing knowledge and skills. Responsibilities include: provision, as assigned, of nursing care, serve as a mentor to less experienced nurses, development of the interdisciplinary plan of care, patient teaching, and support to family members. Physical Demands – Patient Care: Continuous standing/walking and occasional/intermittent sitting. Continuous use of bilateral upper extremities in fine motor activities requiring fingering, grasping, and forward reaching between waist and shoulder level to handle/operate medical equipment/devices. Frequent reaching above shoulder level and overhead. Frequent forward bending, twisting, squatting and kneeling; occasional climbing. Occasional repositioning and transferring patients weighing up to 200 pounds between bed, chair, and gurney. Occasional lifting and carrying equipment weighing up to 25 pounds. Occasional/intermittent pushing of gurneys, wheelchairs, bed and other medical equipment over tiled and carpeted surfaces. Continuous use of near vision to read medical equipment such as monitoring devices and reading documents and computer screens; hearing and verbal communication to interact with patients, co-workers, and other customers. Key for Physical Demands Continuous 66 to 100% of the time Frequent 33 to 65% of the time Occasional 0 to 32% of the time
