Inpatient/Outpatient: 100% inpatient

Position Overview

Henry Ford Warren Hospital is seeking a dedicated and skilled Nurse Practitioner (NP) specializing in Palliative Care to join our dynamic team. NP either board certified in Palliative Care or planning to become. A multidisciplinary approach to patient care, complex in medical decision, symptom management. The ideal candidate will have a strong clinical background in palliative care, excellent communication skills, and a commitment to delivering high-quality patient care. Joining a team of NPs, MDs, social workers, and Chaplin.

Schedule – 0.5 Split Schedule – Thursday, Friday, Monday, Tuesday, Wednesday, every other week.

Weekend requirement – No

In collaboration with a physician, provides for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions. Responsibilities: Obtains patient history and performs assessment via observation, interview and examination. Orders, performs and interprets diagnostic studies. Performs preventative health assessments, screening, immunizations and patient care.| Provides direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources.| Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources.| Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

Qualifications

NP – Completion of a Nurse Practitioner training program accredited by the Commission on the Collegiate of Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC)
NP – Certification by a certification body recognized by the State of Michigan

Licensure and Certification

Current licensure as a Nurse Practitioner in the state of Michigan, or in process of applying (must have proof of application)
National certification as an NP
Basic Life Support (BLS) certification & ASLC.
ACLS not required Palliative Care
Palliative Care BLS required.
DEA/CSL, required through credentialing.
Palliative Care board certified (or planning to become)

Experience

EPIC Experience, preferred.

Skills

Strong clinical assessment and diagnostic skills
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced, multidisciplinary environment.
Commitment to patient-centered are and continuous professional development.
