GENERAL STATEMENT OF DUTIES AND RESPONSIBILITIES
The primary purpose of the position is to provide the assigned residents with routine daily nursing care in accordance with established nursing care procedures, and as may be directed by the Staff Nurse, Resident Care Coordinator, Assistant Director of Nursing and the Director of Nursing.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Performs tasks necessary to help resident in personal hygiene and in carrying out Activities of Daily Living.
2.Report all changes in the residents’ condition to the Charge Nurse ina timely fashion.
3.Observe and report presence of skin breakdown to prevent pressure ulcers.
4.Record all care provided in SigmaCare (Electronic Medical Record).
5.Use the wristband and EMR to identify residents before administering treatments, serving meals, etc., as necessary.
6.Report all accidents/incidents observed on the shift that they occur.
7.Ensure that the resident’s room is ready for receiving the resident (i.e. bed made).
8.Assist with lifting, turning, moving, positioning and transporting residents into and out of beds, chairs, bath tubs, wheelchairs, lifts, etc.
9.Answer resident calls promptly.
10.Assist residents to walk with or without devices as instructed.
11.Perform Nursing Rehab procedures as instructed, including daily range of motion exercise. Record as indicated
12.Provide care as instructed
13.Assist in preparing the Resident for physical examinations.
14.Check Residents routinely to assure that their personal care needs are being met.
15.Perform foot care, report any findings to nurse.
16.Prepare Residents for meals (i.e.. take to bathroom, wash hands, comb hair, raise bed, position tables, place bibs, take to/from dining room, etc.)
17.Serve food trays. Assist with feeding as indicated (i.e., cutting foods, feeding, assist in dining room supervision, etc.)
18.Record the resident’s food/fluid intake as instructed.
19.Serve between meal and bedtime snacks.
20.Perform after meal care (i.e. remove trays, clean resident’s hands, face, clothing, etc.)
21.Maintain the confidentiality of all Resident care information.
22.Ensure that all nursing care is provided in privacy.
23.Knock before entering the Resident’s room; await response.
24.Pack and log all discharge Resident’s belongings.
25.Responsible for cleanliness of Resident’s environment.
26.Make occupied and unoccupied beds.
27.Make bed check rounds on all assigned Residents every two hours at night and report status to charge nurse.
28.Prepare laundry as per policy on assigned days.
29.Measure and record temperatures, pulse and respirations (TPR’s), as instructed.
30.Assist Resident and family during admission/transfer/discharge.
31.Appropriately communicate with residents, including residents who have sensory loss, memory, cognitive or perceptual impairment.
32.Maintenance of personal belongings such as clothing, dentures, eyeglasses, hearing aids and prostheses.
33.Assist with maintaining bowel regularly, report to nurse absence of bowel movements as per policy.
34.Assist with the care of ostomies including but not limited to colostomy and ileostomy, as per nurse’s instructions.
35.Recognize and report abnormal signs and symptoms of common diseases and conditions.
36.Care of the dying resident including care of the body and personal effects after death.
37.May perform other duties and assist in areas of the department as directed by the Director of Nursing, Assistant Director of Nursing, Resident Care Coordinator, Administrator and/or designee.
PHYSICAL, EMOTIONAL AND SENSORY REQUIREMENT

-Must be in good mental and physical condition.
-Must be able to move intermittently throughout the work day.
-Must be able to cope with the mental and emotional stress of the position.
-Must function independently, have flexibility, personal integrity, and the ability to work with the residents, staff, family members, visitors and support and regulatory agencies.
-Must be in good general health and demonstrate emotional stability.
-May be required to lift, push and move at least 50 lbs.
-Must be able to assist in the evacuation of residents.
-Must be able to climb several flights of stairs in an emergency.
-Must maintain a high level of personal hygiene and cleanliness.
-Must be able to physically operate nursing home specific equipment (i.e. hospital bed, Hoyer lift, century tub, etc)
QUALIFICATIONS:
The certified nurse aide must be good moral character. Some geriatric experience is preferable but not essential. Must possess a high school diploma or GED. Must possess a current NYS nurse aide certification and be listed in the New York State RHCF Nurse Aide Registry. Must be able to read, write and speak the English language in an understandable manner to facilitate effective communication and follow oral and written instructions. Must be willing to cooperate with Resident Care Coordinators, fellow employees, other department heads and family members.
SUPERVISOR:
Staff Nurse, Resident Care Coordinator, Assistant Director of Nursing, and Director of Nursing.
WORKING HOURS:
Works 37 1/2 hours per week or as assigned
May work different shifts, i.e.
Works weekends as per facility policy.
6 - 2 PM; 7 - 3 PM
3 - 11 PM, 11 – 7 AM, etc
May work extended hours when necessary .
WORKING CONDITIONS:
-Works throughout the facility, including nursing unit, resident rooms, dining areas, outdoor patio areas, etc.
-Sits, stands, bends, lifts, and moves intermittently, during working hours
-Is subject to frequent interruptions.
-Is involved with residents, personnel, visitors, government agencies/personnel, etc, under all conditions and circumstances.
-Is subject to hostile and emotionally upset residents, family members, visitors, etc.
-Works beyond normal duty hours, and on weekends in an emergency situation (i.e. a snowstorm).
-Attends and participates in continuing educational programs.
-Is subject to falls, odors, etc., throughout the work day.
-May be exposed to residents with chronic and acute conditions and/or illnesses.
COVID-19 considerations:
To keep all staff safe in the facility, all incoming staff are screened by temperature check, and distribution of facemasks daily.
MUST BE FULLY VACCINATED TO BE EMPLOYED AT THIS FACILITY! Proof of vaccination is required.
