Status: non-exempt/full-time
Shift: Monday-Friday/8:00am-4:30pm
Primary Purpose:
- To coordinate delivery of services to managed care and Medicare residents in collaboration with the facility's team members.
- Assist the Director of Clinical Services with ensuring that all documentation in the facility meets federal and state, certification guidelines.
- Coordinate the entire RAI process assuring the accuracy, timeliness, and completeness of the MDS, RAPS, and Interdisciplinary Care Plan.
- Monitor and document the cost effectiveness of treatment provided, facilitates and coordinates the admission and discharge process, serves as the resident and family advocate, and acts as a liaison to insurance and medical management professionals.
Major Responsibilites:
- Complete accurate and timely MDSs, CAAs, and plans of care in accordance with the RAI guidelines for all residents in the facility.
- Provide resident status updates as needed to appropriate personnel.
- Comply with all of the duties and responsibilities noted for the MDS position.
- Perform other duties as assigned.

- Must be able to read, write, speak, and understand the English language.
- Must possess excellent interpersonal and communication skills.
- Must demonstrate the knowledge and skills necessary to prove care appropriate to the age-related needs of the residents served.
- Must be a supportive team member, contribute to, and be an example of teamwork and team concept.
- Must be knowlegeable of nursing and medical practices and procedures, as well as, laws, regulations, and guidelines that pertain to nursing care facilities.
Working Conditions:
- Works in office area(s), as well as, throughout the nursing service area.
- Moves intermittently during work hours.
- Is subject to frequent interruptions.
- Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
- Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
- Is subject to call back during emergency conditions (severe weather, evacuation, post-disaster, etc.)
- Attends and participates in continueing educational programs.
- Is subject to exposure to infectious waste, diseases, conditions, etc., including TB, AIDS, and Hepatitis B viruses.

Minimum Qualifications:
- Must possess a current, unencumbered, active state license to practice as an (RN or LPN).
- Experience in Skilled Nursing/Rehabilitation facilities, preferred.
- Six (6) months of experience as a MDS Coordinator, preferred.
- RAC-CT or RNAC, preferred.
