PURPOSE OF YOUR JOB POSITION
The primary purpose of your job position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by nursing assistants in accordance with facility and through the Mission and Vision, comply with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing or Unit Manager to ensure that the highest degree of quality care is maintained at all times.
II. JOB FUNCTIONS

A. Administrative Functions
- Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the facility.
- Insure that the written policies and procedures that govern the day-to-day functions of the nursing service department are followed by all nursing personnel assigned to your unit.
- Cooperate with other resident services when coordinating nursing services to ensure that the resident's total regimen of care is maintained.
- Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
- Participate in surveys made by authorized government agencies.
- Maintain the Daily Census Report and submit to the Business Office if required.
- Present a positive image of the facility to all customers, such as job applicants, families, and the community.
- Deal honestly, openly, and directly at all times with all Residents and Facility personnel, expressing your views in a positive fashion.
B. Charting and Documentation Functions
- Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures.
- Fill out and complete all forms in accordance with established procedures, including recording Kardex, treatment/care plans, new/changed orders, dietary changes and forward to the appropriate departments as required.
- Assure that appropriate documentation concerning authorized/ unauthorized discharges is entered in the resident's medical record and census sheet in accordance with established procedures.
- Receive telephone and/or tiger text orders from physicians and record and transcribe as required.
C. Drug Administration Functions
- Prepare and administer medications as ordered by the physician.
- Ensure that an adequate supply of all medications, supplies, and equipment are on hand to meet the nursing needs of the residents.
- Dispose of drugs and narcotics in accordance with established facility procedures and ensure that narcotic records are accurate and signed for by your shift.
- Review medication/administration records for completeness of information, accuracy in the transcription of the physician's order, and adherence to stop order policies.
D. Personnel Functions
- Respond to the staffing needs when assigned personnel fail to report to work.
- Develop work assignments, and/or assist in completing and performing such tasks.
- Make daily rounds of your unit to ensure that nursing service personnel are performing their work assignments.
E. Nursing Care Functions
- Administer professional services according to accepted standards of clinical practice.
- Admit, transfer and discharge residents as necessary.
- Participate in the orientation of new residents/family members to the facility.
- Inform nursing personnel of new admissions, their expected time of arrival, room assignment, etc.
- Make rounds with physicians as necessary. Establish resident care, treatment, and rehabilitation.
- Arrange for and schedule any diagnostic and therapeutic services as ordered by the physician.
- Meet with residents, and/or family members, as necessary, including the resident's attending physician and next-of-kin if there is incident/accident or change in the resident's condition.
- Assist in arranging transportation for residents as necessary.
- Check foods brought into the facility by the resident's family/visitors to assure that it is within the resident's dietary allowances.
- Inspect the nursing service treatment areas daily to ensure that they are maintained in a clean and safe manner.
- Ensure resident safety.
- Complete nursing assessment forms upon resident's admission as directed.
F. Staff Development Functions
- Assist with and conduct orientation of new and current employees to your shift. Help new employees understand the importance of great customer service as expected by our facility.
- Attend and participate in continuing educational programs designed to keep you abreast of changes in your profession, as well as to maintain your license on a current status.
G. Safety and Sanitation Functions
- Monitor personnel to assure that they are following established safety regulations in the use of equipment and supplies.
- Ensure that work areas (i.e., nurses' stations, medicine preparation rooms, resident care rooms and treatment areas, etc.) are maintained in a clean and sanitary manner.
- Follow established Infection Control Universal Precautions policies and ensure that your assigned personnel follow said policies, such as:
- Participate in the implementation and maintenance of the infection control program for monitoring communicable and/or infectious diseases among the residents and personnel.
- Participate in the implementation and maintenance of the procedures for reporting hazardous conditions or equipment.
- Ensure that your assigned personnel follow established hand washing techniques in the administering of nursing care procedures.
H. Care Plan Functions
- Review care plans to ensure that appropriate care is being rendered and appropriate changes made to the care plan.
- Review resident care plans for appropriate resident goals, problems,
approaches, discharge plans and revisions based on nursing needs.
I. Resident Rights Functions
- Maintain CONFIDENTIALITY of all resident information.
- Ensure that the residents' rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights are well established and maintained at all times.
- Encourage the utilization of our grievance form for the resident's/family complaints and/or grievance that indicates the action taken to resolve the complaint.
J. Miscellaneous Functions
- Assist in planning clinical supervision for nurse aides in orientation.
- Provide data to the Quality Assurance & Process Improvement Committee as requested.
- Ensure that nursing personnel follow established procedures in the use and disposal of personal protective equipment.
- Accept the duties of being in charge of a unit as assigned to do so.
III. WORKING CONDITIONS
- Sits, stands, bends, lifts and moves during working hours.
- Must be able to move throughout the work day and be on feet for long periods of time and be able to physically move patients.
- Is subject to frequent interruptions.
- Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
- Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
- Works beyond normal working hours, and/or days and in other positions temporarily when staffing numbers indicate the need to meet the regulatory guidelines.
- Attends and participates in continuing educational programs.
- May be exposed to infectious waste, diseases, conditions, etc. including the potential exposure to the AIDS and Hepatitis B viruses and TB.
- Is subject to odors throughout the work day.
- May be required to assist in evacuation of residents.
IV. EDUCATION
Must be, as a minimum, a graduate of an approved LPN/LVN program, or an accredited RN program.
V. PERSONAL, INTERPERSONAL AND COMMUNICATION REQUIREMENTS
- Must possess a current, unencumbered license to practice as a RN or LPN/LVN in this state.
- Must be able to read, write and speak the English language in an understandable manner.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
- Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
- Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
- Must function independently, have flexibility, personal integrity, and the ability to work effectively and tactfully with residents, personnel, visitors, the general public and outside agencies.
V. REPORTING REQUIREMENTS
Reports directly to the Unit Manager and/or the Director of Nursing.
