Lead Resident Care Supervisor (RN) Overview
The Lead Resident Care Supervisor (RN) will be responsible for the clinical oversight, and coordination of the NYS Licensed Home Care Services Agency (LHCSA)/Assisted Living Program (ALP), including UAS assessment for one or more locations, in conjunction with administrative staff and other service agencies.

Responsible for the coordination of ALP program requirements for eligible residents at one or more facilities. Travel to multiple sites will be required.
- Ensure completion of a medical evaluation within 30 days prior to date of admission
- Participate in pre-admission interviews and assessment screenings for prospective residents
- Ensure completion of a mental health evaluation for any prospective resident with a known history of chronic mental disability, or if the medical evaluation or resident interview suggests such disability may exist.
- Complete a UAS-NY Community Assessment or PRI (determined by eligibility) upon admission and every 6 months thereafter, as required by site(s).
- Complete an individualized plan of care for ALP eligible residents upon admission, documentation of HHA and LPN orientation to the plan of care, and continuously maintain an up to date plan of care reflective of the resident’s preferences and care needs.
- Abide by all State and Federal Regulations.
- Maintain safe conditions for residents and staff.
- Participates in nursing on Call rotation of the facility.
- iDemonstrates knowledge and understanding of all policies and procedures and ability to reference them.
Responsible for responding timely to residents’ requests.
- Performs assigned duties according to the service excellence standards and goals established for this position, and for all staff members of this facility.

- Ensures rights of residents and personal preferences in care and treatment are respected and documented on their plan of care.
- Receive and store lost/found articles in cooperation with Administrator or designee.
Assist in the supervision of nursing care services and staff on all med rooms/units/floors while on duty.
- Conduct periodic rounds, provide input for the nursing care of residents, act as a nurse resource person for all nursing staff.
- Monitor resident care standards during performance of duties and ensures that high standards of care are being provided.
- Assist with problem-solving related to staffing, supplies and resident care.
- Appropriately delegates and assigns direct and indirect patient/resident care activities to nursing staff.
- Appropriately revises assignments based on changes in patient/resident census, acuity, and available resources.
- Ensures that nursing staff complete all assignments, promptly report changes in patient/resident conditions, appropriately perform required documentation and comply with all established policies and procedures of the facility and the nursing department
- Interpret personnel policies and procedures to nursing staff, upon request.
- Observes job performance of nursing staff and provides direct feedback.
- Performs timely and accurate documentation according to the service excellence standards and goals established for this position.
- Ensures and/or assists in performing accurate, thorough, and timely completion of nursing admission evaluations and re-evaluations of patient/resident care needs in accordance with nursing department standards of practice.
- Document as applicable in the care plans, nursing notes, medical administration records, treatment records and other required records according to the service excellence standards and goals established for this position.
- Provide evaluation of needs and resident care planning information as necessary and requested by other health care disciplines, completes patient education in accordance with established procedures.
- Contact MD/NP/PA for orders or updates according to patient/resident response to treatment plan and/or situation; update family members as needed.
- Participate in Morning Report and Change-of-Shift Report.
- Is responsible for bringing nursing personnel of the next shift up to date about the care and needs of residents.
Actively promotes controlled substance and facility safety programs.
- Accept and distribute controlled substances to appropriate med rooms/units/floors.
- Assist with the destruction of wasted or outdated controlled substances following the bureau of narcotic rules and regulations/as directed by the DON.
- Performs all position duties according to safety rules and standards of practice required for this position, and as required to ensure the general safety of staff, residents, and visitors of this facility.
- Actively promotes facility safety programs and serve as mentor/contact person as assigned for staff on leave, etc. to ensure strong communication between the department and the staff members.
- Performs first aid services and follows up on Accident/Incident Report of ALP residents, staff and visitors as needed.
- Implements emergency procedures in a correct and efficient manner.
- Report to the attending physician any unusual symptoms, changes in condition, daily needs, and progress of residents.
- Actively participates in the complaint/grievance resolution process by completing requested investigative process and documenting findings in a thorough, timely and concise manner.
- Notify Director of Nursing and Administrator of any significant events/urgent matters related to residents, visitors, staff or any other disruption to facility services.
- Assume responsibility for nursing services in the absence of the Director of Nursing.
- Will be required to take calls on off hours and adjust schedule to meet the needs of the facility.
- Arrives to work on time, regularly and work as scheduled; responsible for own transportation.
- Recognizes and follows the dress code of the facility including wearing name tag at all times.
- Attends facility meetings, staff meetings and required or preferred in-services.
- Participate in Quality Improvement programs (QAPI) upon request.
- Encourages and participates in the ongoing education and professional development of assigned nursing staff, collaborating with appropriate departments and external organizations regarding education programs.
Ability to participate in facility on-call program which may include working at facility on evenings and weekends.
Utilizes electronic timekeeping system as directed.
Arrives to work on time, regularly, and works as scheduled.
Recognizes and follows the dress code of the facility including wearing name tag at all times.
Follows policy and procedure regarding all electronic devices, computers, tablets, etc.

Qualifications
Lead Resident Care Supervisor (RN) Educational Requirements/Essential Job Functions
Graduate of an accredited School of Nursing
Valid and current New York State Registered Nurse License and Registration in good standing in state which work is performed
Minimum of three years nursing experience with one year of home care experience in a LHCSA, CHHA or LTHHCP preferred.
Must possess or be willing to obtain NYS UAS assessor certification, and PRI Training
Previous experience in nursing education, training, and/or performing in-services desired
Demonstrated proficiency in various computer applications, programs, and electronic medical records
