In this role you will be responsible for the coordination of care on all the nursing units in skilled community in accordance with current applicable federal, state and local standards. This position will ensure that Senior Lifestyle standards for resident care are met.

Job Description

Gorham House is a multi-level community with a 68 bed skilled, nursing and mental health component that is looking for an RN with leadership skills to join our dedicated, results driven team!

Ensure, with assistance of the Staffing Coordinator, that appropriate and adequate staffing is in place on all nursing units, all shifts, every day.
Complete and maintain all reports, i.e. I/A logs, monthly QA log, e-Interact, etc. and submit timely to corporative representative as required. Chair/attend meetings to include the weekly At Risk, weekly MC, care plan as per invitation, daily morning stand-up and clinical, etc.
Consult with the ADON and Charge Nurse/Unit Manager to identify, investigative, educate, and discipline nurses related to care or skill issues as indicated.
Oversee the clinical processes/systems in the facility to ensure that practice is maintained as per Senior Lifestyle standards, policies & procedures and within state and federal regulatory guidelines. Manage the survey process when present in the facility.
Coordinate with the Admissions Coordinator to review all potential admissions identifying potential care, behavioral, family issues and the facility’s staff ability to care for.
Monitor current health records, follow-up on incidents and issues with skilled residents and/or families, and schedule meetings as needed.
Follow-up as indicated with falls, infection control, skin issues, unusual occurrences, medication delivery error, and any other issues requiring the attention of a senior administrative member.
Ensure, review, and follow-up with quality assurance parameters including chart audits, log audits, and the scheduling of in-services as indicated.
Develop and maintain a working relationship with professional providers of care to skilled residents both within and outside the facility.
Maintain a professional and informative relationship with both the NHA and ED of the community, keeping them abreast of nursing issues as they develop and any potential impact on the facility.

Qualifications

Registered Nurse with a history in long term care.
Knowledge and experience with Medicare, Medicaid, case mix and rehab/sub-acute services.
Demonstrated background in financial management, policy/procedure implementation, and ability to manage all aspects of the nursing department.
Must have excellent communication, customer service and organizational skills.

Level of Formal Education: Associate or Bachelor’s Degree
Years of Experience: 5-10+ years
Type of Experience: Direct resident care experience, previous experience in supervisory position
Special Certifications: Previous experience in similar position, i.e. Supervisor, ADON, DON at another organization
Language Skills: Acceptable fluency in English in order to perform job duties and speak, read, write, and communicate with all others.
Technical Competencies: Excellent computer skills
Information Systems: Point Click Care and computer skills
Personal Attributes: Professional, organized, experienced, easy going, low-key, confidence in self-knowledge
