Long-Term Care MDS Care Plan Coordinator Opportunity! LPN/RN

Responsibilities of the MDS Coordinator: The MDS Nurse is an LPN or RN who is responsible for the overall supervision, coordination, maintenance, and timely completion of the MDS, care plans, and medical records for residents.
- Oversee and participate in coordinating and completing the Resident Assessment Instrument (MDS and Care Plan) in accordance with current Federal and State Regulations.
- Coordinates, conducts, and completes the MDS, resident care plans, and conferences on the computerized system.
- Assist in the completion of the Resident Assessment Instrument with the Interdisciplinary Team.
- Notify all Interdisciplinary Team members of the MDS Assessment schedule for all payer sources.
- Lead or participate in Daily meetings, weekly Medicare meetings, and month-end meetings to ensure federal billing requirements are met.
- Encourages rehabilitative techniques in order to restore residents' independence.
- Information to complete the MDS is to be collected using the medical record, bedside assessment, and staff, resident, and/or family interviews.
Requirements of the MDS Coordinator RN/LPN:
- Current RN / LPN License
- Long-term care experience preferred
- Current CPR
- At least one (1) year of experience as an MDS Coordinator
